摘要
目的探究临床治疗进展期胃癌患者的过程中,应用腹腔镜辅助胃癌D2根治术治疗对患者免疫功能、细胞因子的影响,并评价其预后价值,为临床胃癌治疗奠定基础。方法选取我院2015年6月~2017年1月期间收治的85例进展期胃癌患者为本研究对象,对比患者应用不同手术治疗措施进行治疗对患者免疫功能、细胞因子及预后的影响。结果予以两种不同的手术治疗方案后,试验组患者的出血量(87.82±18.96)mL、术后肛门排气(3.19±1.27)d及住院时间等各项围手术指标均显著优于对照组,差异有统计学意义(P<0.05)。试验组患者的CD3^+、CD4^+、CD4^+/CD8^+等免疫功能及IL-6、IL-10、TNF-α、TNF-γ等细胞因子改善幅度均显著优于对照组,差异有统计学意义(P<0.05)。且试验组患者的无进展及中位生存时间显著长于对照组,差异有统计学意义(P<0.05)。结论临床治疗进展期胃癌患者的过程中,应用腹腔镜辅助胃癌D2根治术治疗可显著提升患者的免疫功能,改善细胞因子的含量,并有助于延长生存期,应广泛推广。
Objective To explore the effect of laparoscopic assisted D2radical gastrectomy for gastric cancer on immune function and cytokines in patients with advanced gastric cancer,and to evaluate its prognostic value,so as to lay the foundation for clinical treatment of gastric cancer.Methods Eighty-five patients with advanced gastric cancer treated in our hospital from June2015to January2017were selected as the subjects of this study.Effects ofdifferent surgical treatments on immune function,cytokine and prognosis in patients were compared.Results After two different surgical treatments,the bleeding volume(87.82±18.96)mL,postoperative anal exhaust(3.19±1.27)d and hospitalization time of the patients in the experimental group were significantly better than those in the control group,the difference was statistically significant(P<0.05).The immune function of CD3+,CD4+,CD4+/CD8+and IL-6,IL-10,TNF-alpha,TNF-γin the experimental group were significantly better than those in the control group,the difference was statistically significant(P<0.05).The non progression and median survival time of the patients in the experimental group were significantly longer than those in the control group,and the difference was statistically significant(P<0.05).Conclusion In the process of clinical treatment of advanced gastric cancer patients,the application of laparoscopy assisted D2radical gastrectomy for gastric cancer can significantly improve the immune function of patients,improve the content of cytokines,and help to prolong the survival time,should be widely promoted.
