血清肿瘤标志物联合检测在肺癌诊断中的价值研究 被引量:18

摘要 目的探讨血清肿瘤标志物联合检测在肺癌诊断中的价值。方法采用电化学发光免疫分析法检测于该院住院的86例肺癌患者、92例肺部良性病变患者及96例健康体检者血清中神经元特异性烯醇化酶(NSE)、癌胚抗原(CEA)、鳞状细胞癌抗原(SCCAg)、糖类抗原125(CA125)及细胞角蛋白片段21-1(CYFRA21-1)5种肿瘤标志水平,对比各种肿瘤标志物在不同患者、肺癌不同病理分型间的差异,比较肿瘤标志物单项及联合检测在肺癌诊断中效能的差异。结果肺癌组患者血清中NSE、CEA、SCC、CA125及CYFRA211水平均高于肺良性病变组与健康对照组,差异均具有统计学意义(P<0.05)。NSE在小细胞肺癌患者血清中平均水平较高,CEA、CA125在腺癌患者血清中水平较高,SCC、CYFRA211在鳞癌患者血清中水平较高。血清肿瘤标志物联合检测时敏感度均高于单项检测,其中NCE+CEA+CYFRA211具有较高的特异度和准确度,而血清5项指标联合检测虽然灵敏对最高,但其特异度较低,仅为81.53%。结论单项肿瘤标志物检测存在一定的局限性,对于不同病理类型肺癌鉴别诊断的敏感度、特异度存有较大差异,而联合各项血清肿瘤标志物可相互协调互补,能有效提高敏感度及准确度。 Objective To explore the value of combined detection of serum tumor markers in the diagnosis of lung cancer.Methods Electrochemiluminescence immunoassay was used to detect5kinds of tumor markers in86patients with lung cancer,92patients with benign lung disease and healthy persons,serum levels of neuron specific enolase(NSE),carcinoembryonic antigen(CEA),squamous cell carcinoma antigen(SCCAg),carbohydrate antigen125(CA125)and cytokeratin fragment211(CYFRA211).The difference of tumor markers in different patients and different pathological types of lung cancer and the difference of single and combined detection of tumor markers in the diagnosis of lung cancer were compared.Results The serum levels of NSE,CEA,SCC,CA125and CYFRA211in patients with lung cancer were significantly higher than those in benign lung disease group and healthy control group,and the difference was statistically significant(P<0.05).The average level of NSE in the serum of patients with small cell lung cancer was higher,CEA,CA125levels in the serum of patients with adenocarcinoma was higher,and the levels of SCC and CYFRA211in the sera of patients with squamous cell carcinoma were higher.The combined detection of serum tumor markers is more sensitive than that of the single detection,and the combined detection of NCE,CEA and CYFRA211has relatively high specificity and accuracy.The combined detection of5serum indexes has the highest sensitivity,but its specificity is low,only81.53%.Conclusion The single tumor marker has certain limitation whose sensitivity and specificity have great differences for differential diagnosis of different pathological types of lung cancer,and combined detection of the serum tumor markers can be mutually complementary,which could have high sensitivity and accuracy.
