实时三维超声心动检测在风湿性二尖瓣狭窄患者左心室功能评估中的应用价值 被引量:1

Application of real-time three-dimensional echocardiography in left ventricular myocardial segment parameters in evaluating left ventricular function in patients with rheumatic mitral stenosis

摘要 目的分析实时三维超声心动图(RT-3DE)左心室心肌节段参数检测在风湿性二尖瓣狭窄(RMS)患者左心室功能评估中的应用价值。方法回顾性分析焦作市第二人民医院收治的42例RMS患者为观察组,选取同期42例健康体检者为对照组。两组均行二维超声心动图(2DE)、RT-3DE检查,记录两组左心室收缩期末容积(LVESV)、左心室舒张期末容积(LVEDV)、左心室射血分数(LVEF),记录两组左心室基底段前壁、中间段前壁的面积应变值、横向应变值、纵向应变值。结果 RT-3DE检查下两组LVEF水平均低于2DE检查所示LVEF水平,差异有统计学意义(P<0.05);RT-3DE检查下观察组、LVEF、LVESV、LVEDV水平均低于对照组,差异有统计学意义(P<0.05);RT-3DE检查显示两组基底段前壁面积应变值、横向应变值、纵向应变值对比,差异有统计学意义(P<0.05);两组中间段前壁横向应变值、纵向应变值对比,差异有统计学意义(P<0.05)。结论RT-3DE心肌节段参数检测可准确评价RMS患者左心室结构与功能变化,具有较高诊断价值。 Objective To evaluate the application value of left ventricular myocardial segment parameters in the assessment of left ventricular function in patients with rheumatic mitral stenosis(RM S)by real-time threedimensional echocardiography(RT-3DE).Methods A total of42patients with RMS(observation group)and42healthy subjects(control group)were enrolled.Both groups received two-dimensional echocardiography(2DE),and RT-3DE examination.Left ventricular systolic volume(LV ESV),left ventricular end diastolic volume(L V E D V),left ventricular ejection fraction(LVEF)of the two groups were recorded,and left ventricular basement anterior wall,middle section of the anterior wall area strain value,lateral strain value,longitudinal strain value were recorded as well.Results The levels of LVEF were significantly lower in the RT-3DE examination than those in the2-ED examination(P<0.05).The results of RT-3DE showed that the levels of LVEF,LVESV and LVEDV in the observation group were lower than those in the control group(P<0.05).RT-3DE examination revealed that the middle section of the front wall transverse strain value,longitudinal strain value of the two groups had significant difference(P<0.05).Conclusion RT-3DE myocardial segment parameters can accurately evaluate the left ventricular structure and function changes of RMS patients.It is of high diagnostic value.
