摘要
目的评估痛风人群健康生命质量及其影响因素。方法纳入320例痛风患者,采用36条目简明量表(SF-36)评估健康生命质量,比较痛风患者与中国台湾常模的普通人群、慢性期与非慢性期痛风患者间的健康生命质量差异,并分析痛风患者健康生命质量的影响因素。结果共回收306份有效问卷,痛风患者的SF-36生理健康总分(PCS)均低于同年龄段的普通人群(P均<0.01),35岁以上痛风患者心理健康总分(MCS)均低于同年龄段的普通人群(P均<0.05)。慢性期痛风患者PCS、MCS均低于非慢性期患者(37.5±0.3 vs.42.3±0.4,P<0.01;40.4±0.5 vs.44.3±0.4,P<0.01),SF-36的8个维度评分亦均低于非慢性期患者(P均<0.01)。多因素分析显示,目前疼痛视觉模拟评分(VAS)、有吸烟史者比例、合并高血压病者或肾结石者比例、总体医师评价与痛风患者的PCS呈负相关(P均<0.01);总体医师评价、肾功能不全者比例与MCS评分呈负相关(P均<0.05)。结论痛风患者尤其慢性期患者的健康生命质量低下,痛风人群的生命质量主要受痛风发作时VAS、吸烟史、总体医师评价和合并高血压病、肾结石、肾功能不全等因素影响。
Objective To assess the health-related quality of life and its influencing factors in gout patients by SF-36health status questionnaire(SF-36).Methods The health-related quality of life in320gout patients was evaluated by SF-36,which was statistically compared with that of the healthy individuals from Taiwan,chronic and non-chronic gout patients.The influencing factors of health-related quality of life were analyzed.Results A total of306valid questionnaires were retrieved.The scores of physical component summary(PCS)in gout patients were significantly lower than those in the healthy counterparts of the same age(all P<0.01).The scores of mental component summary(MCS)in gout patients aged>35years were significantly lower compared with those in the healthy population(all P<0.05).The PCS/MCS scores in chronic gout parents were significantly lower than those in the patients with non-chronic gout(37.5±0.342.3±0.4,P<0.01;40.4±0.544.3±0.4,P<0.01).The scores of8scales of SF-36in the chronic gout patients were significantly lower compared with those in the non-chronic gout counterparts(all P<0.01).Multiple linear stepwise regression model demonstrated that visual analogue score(VAS),the percentage of smoking history,the percentage of hypertension or kidney stone and physician global assessment were negatively associated with PCS score(all P<0.01).Physician global assessment and the percentage of kidney insufficiency were negatively associated with MCS score(both P<0.05).Conclusions The health-related quality of life in gout patients,especially in those with chronic gout,is low.The VAS upon gout episode,smoking history,physician global assessment,hypertension,kidney stone and kidney insufficiency are the influencing factors of the health related quality of life in gout patients
