摘要
目的观察益心舒胶囊治疗矽肺合并肺动脉高压的临床疗效。方法 71例矽肺合并肺动脉高压患者随机分为对照组35例和观察组36例。对照组给予抗炎、止咳化痰平喘等常规治疗,观察组在其基础上加用口服益心舒胶囊,观察两组治疗前后肺动脉压力值、血气分析指标[动脉血氧分压(PaO_2)、动脉血二氧化碳分压(Pa CO2)、动脉血氧饱和度(SaO_2)]、肺功能指标[最大通气量(MVV)、第1秒用力呼气容积/用力肺活量(FEV1/FVC)、第1秒用力呼气容积(FEV1)]、不良反应情况及临床疗效。结果观察组总有效率为86.11%,明显高于对照组的54.29%,差异具有统计学意义(P<0.05)。治疗后两组SaO_2、PaO_2均有改善,观察组改善更明显(P<0.05);两组Pa CO2改善不明显,比较差异无统计学意义(P>0.05)。两组MVV、FEV1、FEV1/FVC等肺功能指标均改善,观察组效果更明显(P<0.05)。两组的肺动脉平均压均下降,观察组下降更明显(P<0.05)。治疗期间两组均未发生不良反应。结论益心舒胶囊治疗矽肺合并肺动脉高压的临床疗效肯定,能改善肺功能和降低肺动脉压力,无严重不良反应,值得推广。
Objective To observe clinical effect by Yixinshu capsules in the treatment of silicosiscomplicated with pulmonary arterial hypertension.Methods A total of71patients of silicosis complicated withpulmonary arterial hypertension were randomly divided into control group with35cases and observation groupwith36cases.The control group received conventional therapy by anti-inflammation,relieving cough,reducingsputum,and relieving asthma.The observation group received additional oral administration of Yixinshu capsuleson treatment for the control group.Observation was made on pulmonary arterial pressure,blood gas analysisindexes[arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2),arterialoxygen saturation(SaO2)],pulmonary function indexes[maximal voluntary ventilation(MVV),forced expiratoryvolume in1second/forced vital capacity(FEV1/FVC)],adverse reactions and clinical effects before and aftertreatment in the two groups.Results The observation group had obviously higher total effective rate as86.11%than54.29%in the control group,and their difference had statistical significance(P<0.05).Both groups hadimproved SaO2and PaO2after treatment,and the observation group had more remarkable improvement(P<0.05).Improvement of PaCO2was not obvious in both groups,and their difference had no statistical significance(P>0.05).Both groups had improved MVV,FEV1,FEV1/FVC indexes of pulmonary function,and the observation group hadmore remarkable improvement(P<0.05).Mean pulmonary arterial pressure was decreased in both groups,and the observation group had more obvious decreased level(P<0.05).There was no adverse reaction in the two groupsduring treatment.Conclusion Yixinshu capsules shows affirmative clinical curative effect in the treatment of silicosis complicated with pulmonary arterial hypertension.This method can improve pulmonary function andlower pulmonary arterial pressure,without and severe adverse reactions.It is worth promoting.
