摘要
目的 :探讨治疗口止母外翻的最佳手术方法。方法 :在 Mc Bride矫形术的基础上 ,我们采用拇收肌切断不做后移固定 ,外侧籽骨摘除 ,第 1、2跖骨头处关节囊或骨膜拉紧缝合 ,跖趾关节内侧舌形瓣重叠缝合 ,切开第 1跖跗内侧关节囊 ,做皮下缝合切口。随访 0 .5～ 4年。结果 :我们采用北医一院骨科的疗效评定方法 ,分优良可。优 5 2例 98足 ,占82 % ;良 12例 2 0足 ,占 18% ;可无。此组病例患者无一例发生手术后并发症。结论 :改良后的 Mc Bride矫形术是治疗口止母外翻畸形的最佳手术方法 ,它不做截骨 。
Objective:Research the best operative method to cure hallux valgus.Metoods:Our operative plan is based on McBride operation,we cut off adductoris hallucis muscle,draw it back and fix it,we also excise the ossa scsamoidca them batween the caputs of the first and the second os matatarsales.We design tongue-shape flaps in the medialis of the joint between os metatarsale (I) and phalanx proximalis sew up and overlap them.We also open the capsula orticularis medialis which is around the first articulationes tarsometatarseae,at last we sew up the incision in tela subcutanes.Follow-up studies between a half to four years were done.Results:According to the standard of orthopaedics of the first hospital in Beijing.In all cases,52 cases were excellent (82%),12 cases were good (18%).No one is fair.There was no complieation in these cases.Conclusion:The improved McBride orthopacdics is the bcst operative way to cure hallux valgus,without the osteotomy the foot heal up fast and the function can resumc early.
