摘要
目的 :总结寰枢椎旋转半脱位的诊治体会。方法 :对本院骨科 1999年 1月～ 2 0 0 1年 7月收治的 2 5例寰枢椎旋转半脱位患者的临床情况、治疗经过进行分析 ,同时参照相关文献探讨目前对该病的诊治进展。结果 :Fieldling 、 型病人可以大部分经早期保守治疗获得治愈 ,两例经后路改进型 Halifax椎板夹复位固定获得良好治愈。结论 :寰枢椎旋转半脱位多发于青少年。发病后得到早期诊断及治疗是关键 ,保守治疗为首选 ,若无效可行寰枢融合术。而改良型
Objective:To summarize the experience in diagnosis and therapy of atlantoaxial rotatory subluxation.Methods:Twenty-five patients with atlantoaxial rotatory subluxtion were reviewed,who were treated in our department form 1999 to 2001.Clinical status and process of therapy were studied.The pertinent literatures was refer to discuss the progress in therapy of atlantoaxial rotatory subluxation.Result:25 patients with Fielding type Ⅰ?Ⅱ of atlantoaxial rotatory subluxation were cured by conservative therapy.Two patients with Fielding type Ⅲ who were failure by conservative therapy required posterior atlantoaxial arthrodesis and were treated with improved Halifax interlaminar clamps and had gotten satisfied clinical outcome.Conclusion:Atlantoaxial rotatory subluxaion are often found in children.The key of therapy was treated in earlier period and conservative therapy is the chief therapeutics.If it is failure,atlantoaxial arthrodesis is requiring and Halifax laminal clamps is safe effective and suiatble for atlantoaxial arthrodesis.
