肾病综合征患儿激素治疗前后外周血单个核细胞凋亡的临床意义 被引量:2

摘要 目的 观察小儿肾病综合征 ( NS)泼尼松治疗前后外周血单个核细胞 ( PBMC)的细胞凋亡变化 ,探讨泼尼松诱导细胞凋亡关系及临床意义。方法  5 0例小儿肾病综合征患者分别泼尼松治疗前和治疗后第二、四、六、八周采血 ,进行细胞凋亡形态观察 ,计算百分率。结果 治疗前凋亡率为 ( 9.9± 6.2 ) % ;第二周为 ( 1 7.3± 5 .83) % ;第四周为 ( 1 5 .6± 4.5 8) % ;第六周为 ( 1 3.6± 6.0 6) % ;第八周 (减药两周后 )为 ( 1 0 .1 8± 4.0 1 ) %。治疗前与治疗后二 ,四 ,六周比较均 P <0 .0 1差异有显著性意义 ;与第八周 (减药两周后 )比较 P>0 .0 5差异无显著性意义。结论 小儿 NS患者使用足量泼尼松治疗可诱导外周血单个核细胞 ( PBMC)细胞凋亡增加 ,也诱导大部分患者病情缓解 ,当泼尼松减量后诱导细胞凋亡随之相对减少。由此可见观察外周血单个核细胞 ( PBMC )的细胞凋亡变化对 Objective To observe the effect on the apoptosis of the PBMC in children suffering from nephrotic syndrome(NS) treated with prednisone, and to explore the relation of apoptosis induced by prednisone and its clinical significance Methods Take blood from fifty patients with NS before and after prednison administration(in the 2nd,4th 6th,8th weeks),then calculate and observe the rate of the apoptosis and their forms Results The rate of the apoptosis before and after prednisone administration is as following: before treatment (9 9±6 2)%;after treatment in the 2nd week (17 3±5 83)%;4th week (15 6±4 58)%6th week (13 6±6 06)% and 8th week (two weeks after reducing the dose)(10 18±4 01)%.The differences before and after administration in the 2nd ,4th,6th weeks are statistically significant (P<0 01),but there are no significant differences before and after administration in the 8th week Conclusions Giving full dose of prednisone to the children suffering from NS not only induce the increase of the apoptosis in the PBMCs, but also remit most of the patients' condition. And the apoptosis also decreases when diminishing the dose of prednison. Monitoring apoptosis of the PBMC is a useful method to guide clinical treatment
