三硝基苯磺酸(TNBS)诱导大鼠胰腺纤维化的病理学演变 被引量:12

摘要 目的 观察三硝基苯磺酸(TNBS)胰管内注射诱导大鼠慢性胰腺炎胰腺纤维化过程中的病理演变规律,进一步从病理学角度揭示其发生机制。方法 通过胰管内注射含2%TNBS的乙醇磷酸盐缓冲液诱导大鼠慢性胰腺炎模型,对照组仅注射等体积乙醇磷酸盐缓冲液,并于术后72h、3周、4周、5周、6周、7周处死大鼠。应用光镜、电镜观察不同时间点胰腺组织的病理学变化。结果 2%TNBS胰管注射后早期主要以胰腺组织炎症、水肿,腺泡细胞坏死等改变为主;3周后则以纤维化为主,主要表现为胰腺星状细胞活化和成纤维细胞增生,腺泡萎缩及间质内大量纤维沉积。结论 胰管内注射TNBS引起的慢性胰腺炎的病理改变是基于胰腺实质急性损伤而发生的胰腺组织再生与修复,最终形成胰腺纤维化。 Objective To observe the progressive histopathologic alteration and pathogenesis in a trinitrobenenze sulfonic acid(TNBS) induced chronic pancreatitis rat model. Methods A chronic pancreatitis model was established by infusing 2% TNBS in ethanolate-phosphate buffer solution into the rat pancreatic duct, and the control group was treated with the equal amount of the buffer vehicle only. The animals were sacrificed at 72 hours, and 3, 4, 5, 6 and 7 weeks after operation. The sequential histopathologic changes were observed by light microscope and electron microscope. Results The early histopathologic changes after injection of TNBS solution mainly included acute inflammation of the pancreas, interstitial edema, infiltration of inflammatory cells and acinar cellular necrosis; by week 3, acinar cellular atrophy, interstitial fibrosis, activation of pancreatic stellate cells and proliferation of fibroblasts were noted. Conclusions Sequential histopathologic alterations from inflammatory response and necresis to pancreatic fibrosis in the TNBS induced chronic pancreatitis rat model support the idea that pancreatic fibrosis may be based upon the changes of pancreatic necrosis.
