摘要
目的 为胫骨骨折治疗探索一种新的内固定技术。 方法 运用微创技术经皮钢板内固定治疗 4 5例胫骨骨折。在胫骨内侧建立皮下隧道 ;通过此隧道将钢板安放在胫骨内侧骨膜上 ,骨折间接复位 ,少数螺钉予以固定。 结果 所有病例得到随访 ,时间 12~ 2 4个月 ,平均 15个月。骨折愈合时间 2 .5~ 4 .5个月 ,平均 3.1个月。未见骨不连及钢板断裂、松动。 结论 微创钢板接骨术能减低对骨折处血运的干扰 ,提高骨折愈合率。
Objective To study a new approach and internal fixation for treatment of tibial fractures. Methods Minimally invasive plate osteosynthesis (MIPO) was used to treat 45 cases of tibial fractures. A subcutaneous tunnel was created along the medial border of the tibial. Axial alignment of the fracture was performed using indirect reduction techniques. After the quality of reduction was verified by fluoroscopy, a longer prebent plate was inserted into the subcutaneous tunnel. The plate using fewer screws was placed onto the extraperiosteal surface to achieve an optimal fixation. The patient was suggested to do early functional exercise after operation. Results All the patients were followed up for 12 24 months after operation (average 15 months). All the bone fractures were healed with an average healing time of 3.1 months without nonunion, breaking or loss of plate. Conclusions Minimally invasive plate osteosynthesis can reduce disturbing vascularization of bone fragment, accelerate bone healing and reduce complication.
