The value of 3D-SPACE sequence in diagnosis of vestibular paroxysmia

摘要 目的探讨可变翻转角的三维快速自旋回波序列(3D-SPACE)对前庭阵发症(VP)的诊断价值。方法应用西门子Skyra 3. 0T磁共振扫描仪3D-SPACE序列对25例前庭阵发症患者(VP组)以及30例其他眩晕患者(对照组)行桥小脑角区扫描,对两组前庭蜗神经周围有无血管神经交互压迫(NVCC)以及压迫类型进行回顾性分析并比较其结果。结果 VP组有22例(88%)患者存在NVCC,其中单侧18例,双侧4例;对照组有11例(36. 7%)患者有NVCC,其中单侧9例,双侧2例,VP组NVCC阳性率明显高于对照组,差异有统计学意义(X^2=14. 972,P <0. 001)。VP组最常见的NVCC的类型为血管袢压迫(12/26,46. 1%),两组间NVCC的分型差异无统计学意义。结论 VP组NVCC阳性率高,压迫类型以袢压迫多见,3D-SPACE序列能很好地显示NVCC及其类型,对VP的诊断有较高的临床价值。 Objective To explore the value of MR 3D sampling perfection with application optimized contrasts using different flip angle evolutions (3D-SPACE) in diagnosis of vestibular paroxysmia. Methods The cerebellopontine angle areas were axially scaned by Siemens Skyra 3.0 T MRI with 3D-SPACE in 25 VP patients (VP group) and 30 other vertiginous patients (control group), the data were retrospectively reviewed for neurovascular cross compression (NVCC) analysis, the types of NVCC were compared between the two groups. Results 22 patients (88%) in VP group existed NVCC,and the unilateral in 18 cases the bilateral in 4 cases. 11 patients (36.7%) in control group existed NVCC, and the unilateral in 9 cases, the bilateral in 2 cases. The NVCC positive rate in VP group was significantly higher than control group ( 2=14.972, P 〈0.001). The most common NVCC type was vascular loop compression at vestibulocochlear nerve (12/26, 46.1%). There were no significant differences between the two groups in the type of NVCC. Conclusion The VP patients have higher NVCC positive rate and the most common NVCC type is vascular loop compression, 3D-SPACE can clear display NVCC and its type, which has high clinical value in diagnosis of vestibular paroxysmia.
