摘要
目的探讨不同剂量普通肝素对胎盘早剥新生儿的临床疗效及安全性。方法将我院胎盘早剥的60例新生儿按随机数字表法分为A组(n=30)和B组(n=30),两组普通肝素干预治疗剂量分别为0.3 mg/kg、0.1 mg/kg。比较两组疗效、肝素使用后大出血发生率、住院时间及凝血指标。结果 A组治愈率显著高于对照组,差异具有统计学意义(P<0.05);两组大出血发生率无显著差异(P>0.05);A组住院时间短于B组,差异具有统计学意义(P<0.05);使用肝素1、2 d后,两组PT及APTT均显著下降,FIB水平均显著升高,且A组优于B组(P<0.05)。结论 0.3 mg/kg普通肝素运用于胎盘早剥新生儿后,可以显著改善新生儿体内高凝状态,同时肝素使用中并未明显增加大出血的发生率。
Objective To investigate the clinical efficacy and safety of different doses of unfractionated heparin in neonateswith placental abruption. Methods The 60 cases of neonates with placental abruption in our hospital were divided into groupA（n=30） and groupB （n=30） according to the random number table method. The doses of unfractionated heparin interventiontreatment in the groups A andB were 0.3 mg/kg and 0.1 mg/kg, respectively. The therapeutic efficacy, the incidence of majorhemorrhage after heparin use, the hospital stays and the coagulation parameters of the two groups were compared. ResultsThe cure rate of the group A was significantly higher than that of the group B, and the difference was statistically significant（P〈0.05）. There was no significant difference in the incidence of major hemorrhage between the two groups（P〉0.05）. Thehospital stay in the group A was shorter than that in the group B, and the difference was statistically significant（P〈0.05）.After using heparin for 1 and2 days, PT and APTT significantly decreased in both groups, FIB levels significantly increased,and those in the group A were better than the groupB （P〈0.05）. Conclusion After using 0.3 mg/kg of unfractionated heparinin neonates with placental abruption, the neonatal hypercoagulability can be significantly improved, and use of heparin doesnot significantly increase the incidence of major hemorrhage.
