sST2、脂联素、生长分化因子-15联合检测对老年心力衰竭的诊断意义 被引量:6

Clinical significance of the combined detection of sST2, adiponectin(APN) and growth differentiation factor-15 (GDF-15) in the diagnosis of senile heart failure

摘要 目的探讨血清标志物s ST2、脂联素(APN)、生长分化因子-15(GDF-15)联合检测对老年心力衰竭的诊断意义。方法采用酶联免疫吸附试验检测150例确诊为老年心力衰竭的不同心功能分级患者(研究组)以及50例非心力衰竭患者(正常组)的血清标志物s ST-2、APN及GDF-15水平。观察单项指标以及联合检测指标的敏感度和特异度。结果研究组血清s ST-2、APN、GDF-15水平均明显高于正常组(P<0.05),随着NYHA分级的升高,患者血清s ST-2、APN和GDF-15水平也相应升高(P<0.05);血清s ST-2+APN+GDF-15联合检测诊断老年心力衰竭的敏感度为91.3%,特异率为86.1%,均高于单项检测者。结论血清s ST-2、APN及GDF-15三项指标联合检测可提高老年心力衰竭诊断的敏感度。 Objective To investigate the clinical significance of the combined detection of serum marker s ST2, adiponectin(APN) and growth differentiation factor-15(GDF-15) in the diagnosis of senile heart failure. 150 senile Methods heart failure patients with different heart function grading were enrolled in the Study Group while 50 cases with no heart failure were enrolled in the Normal Group. Their serum BNP, s ST-2, APN and GDF-15 levels were detected by Enzyme-linked immunosorbent assay. The sensitivity and specificity of individual indicators and indicators of combined detection was observed. The Study Group had the levels of serum s ST-2, APN and GDF-15 significantly higher than the Normal ResultsGroup(P〈0.05). With the increase of NYHA grading, the levels of serum s ST-2, APN and GDF-15 were also increased(P〈0.05); the combined detection of serum s ST-2+APN+GDF-15 had a sensitivity of 91.3% and a specificity rate of 86.1%, which were all higher than the detection of each individual indicator(s ST-2:69.3%; APN:76.6%; GDF-15:78.7%). Conclusion Combined detection of serum s ST-2, APN and GDF-1 can improve the sensitivity in the diagnosis heart failure in the elderly patients, and it is worth promoting.
