摘要
目的探微分析产前B超检查诊断胎儿骨骼肢体畸形的价值。方法回顾分析2018年4—9月期间到该院接受产前B超检查诊断的800名孕妇的临床档案资料,对其胎儿的骨骼形态、肢体结构、运动表现及姿势等进行连续顺序的追踪观察,进而分析诊断其有无骨骼肢体畸形情况,并将筛查出的畸形结果与后期胎儿引产或出生时的骨骼肢体畸形表现进行对比,进而探讨产前B超检查诊断胎儿骨骼肢体畸形的价值。结果 800名孕妇经过产前B超检查诊断显示,胎儿畸形发生率为1.25%(10/800)。经孕妇在产后对比证实,共有11例胎儿发生骨骼肢体畸形,其畸形率为1.38%(11/800),故该次检出率为90.91%(10/11)、漏诊率为9.09%(1/11,漏诊1例多趾儿),而产前B超检查的诊断准确率为100.00%。其畸形种类如:多趾(指)、缺指、前臂缺失(手缺失)、桡骨缺失、四肢短小(股骨不等长)、小腿缺失(足缺失)、足内翻、肢体姿势异常、成骨发育不全、致死性侏儒、人体鱼序列征。结论规范产前B超检查对筛查诊断胎儿的骨骼肢体畸形有显著效果,能够及早发现胎儿的生长发育过程中出现的异常畸形情况,对预防及减少畸形儿的出生有重大意义,对优生也有积极的保障作用,并且减少了家庭负担及社会影响。
Objective To explore the value of prenatal B-ultrasound in the diagnosis of fetal bone deformity. Methods The clinical archives of 800 pregnant women who underwent prenatal B-ultrasound examination in the hospital from April to September 2018 were retrospectively analyzed. The skeletal morphology, limb structure, motor performance and posture of the fetus were continuously sequenced follow-up observation, and then analyze the diagnosis of the presence or absence of skeletal limb malformation, and compare the results of the malformation detected by the screening with the performance of post-natal fetus induction or skeletal limb malformation at birth, and then explore the value of the prenatal B-ultrasound diagnosis of fetal skeletal limb malformation. Results 800 pregnancies diagnosed by prenatal B-ultrasound showed that the incidence of fetal malformation was 1.25% (10/800). After maternal postpartum comparison confirmed that there were 11 eases of fetal limb malformation, the deformity rate was 1.38% (11/800), so the detection rate was 90.91% (10/11), and the missed diagnosis rate was 9.09% (1/11, missed one ease of muhi-toe), and the diagnostic accuracy of prenatal B-ultrasound was 100.00%. The types of malformations were: multi-toe (finger), missing fingers, forearm loss (hand missing), missing humerus, short limbs (femur length), calf loss (foot missing), foot varus, abnormal limb posture, osteogenesis Dysplasia, lethal gnome, human fish sequence sign. Conclusion Prenatal B-ultrasound examination has a significant effect on screening and diagnosing fetal limb malformations. Early detection of abnormal malformations during fetal growth and development is of great significance for preventing and reducing the birth of deformed children, positive protection and reduced family burden and social impact.
