摘要
目的分析药品不良反应发生特点,促进合理用药,减少药品不良反应发生率。方法对辽宁省盘锦市辽河油田总医院2009年1月至2017年12月上报的508例药品不良反应按照年龄、易发人群、药品种类、发生时间、给药途径、转归、累及器官以及临床表现等方面进行分析;同时对药品不良反应的严重程度进行多因素相关分析。结果 508例药品不良反应中50岁以上患者286例,占56.3%;静脉用药占83.6%;用药后24h内发生药品不良反应341例,占67.2%;转归良好,无恶性事件发生;抗感染药物引起的药品不良反应237例,占46.7%;营养类药物和中药分别居第二和第三位;临床表现以皮肤及附件损伤最常见,占总例数的37.6%,其次为全身性损害。体质量对药品不良反应的严重程度影响显著(P<0.05)。结论加强对易发人群用药管理,重点加强给药途径、风险品种、易发部位的监测,控制体质量等可减轻药品不良反应严重程度。
Objective To analyze the characteristics of adverse drug reactions（ADRs）in order to promote rational use and reduce the incidence of ADRs.Methods Case reports of 508 patients with ADRs treated in our hospital from January 2009 to December 2017 were statistically analyzed according to age,susceptible populations,brands of ADR-inducing drugs,time of occurrence,administration routes,prognosis,organs and systems involved in ADRs and clinical manifestations.Multi-factor analysis was conducted of the severity of ADRs.Results The incidence of ADRs was the highest in patients（56.3%）over 50 years old.The incidence of ADRs in the departments of Cadres,Nephrology,Oncology and Hematology was higher than in other departments.Venous transfusion was the main administration route（83.6%）.67.2% of the ADRs occurred within 24 hand the prognosis was good.ADRs induced by antimicrobial drugs accounted for 46.7% of the total,involving 237 patients.The rates of ADRs induced by nutriceuticals and traditional Chinese medicine took the 2 nd and 3 rd place separately.Most of the organs and systems involved in ADRs were the skin and its appendages（37.6%）.Weight could have significant influence on the severity of ADRs.Conclusion Medication among susceptible populations should be made more specific.It is very important to strengthen the monitoring of administration routes,risk varieties and prone spots.Early detection and treatment are essential.Weight control may reduce the severity of ADRs.
