摘要
目的探讨应用泪道探通联合逆行置入泪道引流管手术治疗泪道阻塞的临床疗效以及安全性。方法选择该院2016年3月—2018年3月接受手术治疗的泪道阻塞患者58例,根据其采用的术式不同将其分为探通置管组(患者数40例,患眼52眼)、阻塞切开组(患者数18例,患眼26眼);手术1个月后,对比两组疗效、围术期并发症情况。结果 (1)探通置管组治疗总有效率为90.4%,明显高于阻塞切开组69.2%(χ~2=4.165,P=0.041<0.05);(2)探通置管组出现术中鼻腔出血6例,均经收敛鼻黏膜处理快速止血,并发症率为15.0%,阻塞切开组除术中鼻腔出血外,还出现术后鼻腔感染、局部疼痛、术后出血等并发症,并发症率为50.0%,探通置管组的并发症率也明显低于阻塞切开组(χ~2=6.211,P=0.013<0.05)。结论泪道探通联合逆行植入泪道引流管治疗泪道阻塞的临床疗效满意、并发症少、创伤小,该术式在泪道阻塞的临床治疗中具有重要的应用价值。
Objective To investigate the clinical efficacy and safety of lacrimal passage combined with retrogradeplacement of lacrimal drainage tube in the treatment of lacrimal duct obstruction. Methods A total of 58 patients withlacrimal duct obstruction who underwent surgery from March 2016 to March 2018 in the hospital were enrolled. Ac-cording to the different surgical procedures, they were divided into the probing catheterization group （40 patients, 52eyes）, obstructive incision group （18 patients, 26 eyes）; 1 month after surgery, the efficacy and perioperative complica-tions of the two groups were compared. Results 1.The total effective rate of the treatment group was 90.4%, which wassignificantly higher than that of the obstructive incision group 69.2% （χ2=4.165, P=0.041±0.05）; 2.the probing tubegroup appeared in the 6 cases of intranasal hemorrhage were treated with ablation of nasal mucosa for rapid hemostasis.The complication rate was 15.0%. In addition to intraoperative nasal hemorrhage, there were postoperative complica-tions such as postoperative nasal infection, local pain and postoperative bleeding. The complication rate was 50.0%.The complication rate of the sputum catheterization group was also significantly lower than that of the occlusion group（χ2=6.211,P=0.013±0.05）. Conclusion The clinical efficacy of lacrimal passage and retrograde implantation of lacrimaldrainage tube for lacrimal passage obstruction is satisfactory, less complications and less trauma. This procedure hasimportant application value in the clinical treatment of lacrimal duct obstruction.
