摘要
目的探讨自锁托槽矫正技术在正畸患者中的应用及对龈沟液内白细胞介素(IL)-1β、肿瘤坏死因子(TNF)-α水平的影响。方法研究对象选自我院2014年1月~2016年10月收治的需做固定矫治的72例患者,采取随机数字表将其分成两组,每组36例。其中观察组采用自锁托槽矫治器治疗,对照组采用传统直丝弓矫治器治疗,对比两组治疗前后龈沟液内IL-1β、TNF-α水平变化情况。结果两组治疗后1、2、3个月龈沟液量逐渐增多,且均明显多于治疗前(P<0.01),观察组治疗后1、2、3个月龈沟液量明显少于对照组同时点(P<0.01)。两组治疗后1、2个月龈沟液内IL-1β、TNF-α水平均逐渐升高,治疗后3个月开始回落,但均较治疗前显著升高(P<0.01),观察组治疗后1、2、3个月龈沟液内IL-1β、TNF-α水平明显低于对照组同时点(P<0.01)。结论与传统托槽矫治器治疗相比,自锁托槽矫正技术更有利于维持牙周组织健康,其引起的局部炎症反应更轻。
Objective To explore the efficacy of self-locking brackets in orthodontic patients and its effect on interleukin-1β （IL-1β） and tumor necrosis factor-α （TNF-α） in gingival crevicular fluid. Methods We selected 72 cases those need fixed treatment in our hospital from January 2014 to October 2016 as research objectives and divided them into two groups with 36 cases in each group. Observation group were treated with self-locking brackets while control group were treated with conventional straight wire appliance. We also compared two groups’ levels of IL-1β and TNF-α in gingival crevicular fluid before and after treatment. Results The amount of gingival crevicular fluid increased gradually in both groups at 1st, 2nd and 3rd month after treatment, which were significantly higher than that before treatment （P〈0.01）. Observation group’s gingival crevicular fluid at 1st , 2nd and 3rd month after treatment were significant less than those of control group at the same time （P〈0.01）. The levels of IL-1β and TNF-α in gingival crevicular fluid in both groups increased gradually at 1st and 2nd month after treatment, while droped at 3rd month after treatment but still significant higher than those before treatment （P〈0.01）. Observation group’s IL-1β and TNF-α in gingival crevicular fluid at 1st and 2nd month after treatment were significant lower than those of control at the same time （P〈0.01）. Conclusion Compared with traditional brackets treatment, self-locking brackets correction technology is more conducive to the maintenance of periodontal health and causes less local inflammatory reaction.
