百草枯中毒的机制及诊疗进展 被引量:10

摘要 肺部损伤是百草枯中毒的特征性病变,由此造成的不可逆性肺纤维化和进行性呼吸衰竭是百草枯中毒患者的常见死因。氧自由基的产生和氧化应激反应是目前尚明确的中毒机制,通过已知的症状和体征判断中毒程度,体液中百草枯含量的监测是重要临床诊断依据。至今为止还没有特效的百草枯中毒解毒剂,血液灌流、抗氧化治疗、激素冲击疗法和应用免疫抑制剂等对症治疗是缓解病情进展的有效手段,报道的其他新型治疗手段如血必净等的疗效还需临床进一步观察。 Acute lung injury is a characteristic lesion of paraquat poisoning. The resulting irreversible puhnonary fibrosis and progressive respiratory failure are common causes of death in patients with paraquat poisoning. The production of oxygen free radical and oxidative stress response are currently well-defined mechanisms of the poisoning. Judging the extent of poisoning through known symptoms and signs and the monitoring of paraquat in body fluids are important clinical diagnostic basis. Until now, there are no specific antidote of paraquat poisoning. Blood peffusion, anti-oxidation therapy, hormone shock therapy, inmmnosuppressive agents and other symptonlatie treatment are effective means to alleviate the disease progression, and the efficacy of other new treatments such as Xuebijing needs further clinical observation.
