摘要
目的系统评价我国唇龈沟进路与鼻内镜下揭盖术治疗鼻前庭囊肿临床疗效,为临床治疗鼻前庭囊肿术式选择提供循证依据。方法计算机检索VIP、CBM、CNKI、WanFang、PubMed、EMBASE、Google、Cochrane Library(截止至2017年12期),由两位研究者独立完成文献筛选,纳入关于唇龈沟进路与鼻内镜下揭盖术治疗鼻前庭囊肿的随机对照实验。对其资料进行综合质量评估,应用Revman5.3软件进行Meta分析。结果共纳入19篇文章数据,共有1619例患者,824例患者行唇龈沟进路手术,795例患者经鼻内镜下揭盖术治疗,鼻内镜下揭盖术的手术时间[MD=3.69,95%CI[3.50,3.88];出血量[MD=30.86,9 5%C I [3 0.4 8,31. 2 4];术后2 4小时疼痛[M D=3. 53,95%CI[3.31,3.74];住院时间[MD=2.25,95%CI[2.10,2.41];愈合时间[MD=2.14,95%CI[2.08,2.21];术后复发时间[MD=4.13,95%CI[2.45,6.97];均明显小于经唇龈沟进路手术(P<0.00001)。结论与唇龈沟进路组相比,鼻内镜下揭盖术可明显缩短手术时间、住院时间及愈合时间,减少术中出血量、术后复发率等。鉴于本次研究仅限于国内文献,加之可能存在发表偏倚的影响,尚需更多国内外纵向、横向研究并给予高质量的随机对照试验进行验证。
OBJECTIVE To evaluate the clinical efficacy of Labiogingival groove surgical approach and nasal endoscopy approach in the treatment of nasal vestibular cysts in China, and to provide evidence-based basis for the selection of surgical methods for the treatment of nasal vestibular cysts. METHODS VIP, CBM, CNKI, WanFang, PubMed, EMBASE, Google, Cochrane Library（up to issue 12 in 2017） was searched by computer, and the literature was screened by two researchers independently, which was included in a randomized controlled trial on the treatment of nasal vestibular cysts by labiogingival groove surgical approach and nasal endoscopy approach. For a comprehensive quality assessment of its information, the Meta analysis was carried out with Revman5.3 software. R ESU LTS Nineteen ar ticles were included in this study. There were 1619 patients totally. Labiogingival groove surgical approach was used in 824 patients and nasal endoscopy approach was used in 795 patients. The operative time of endoscopic dissection[MD =3.69, 95%CI[3.50, 3.88], bleeding volume[MD =30.86, 95%CI [30.48, 31.24], postoperative pain[MD =3.53, 95%CI[3.31, 3.74], hospitalization time [MD =2.25, 95%CI[2.10,2.41], The healing time[MD =2.14, 95%CI[2.08,2.21]and postoperative r e c u r r e nc e t i me[M D = 4.13, 95%CI[2.45,6.97 ] we r e significantly shorter than that of the transgingival groove surgical approach（P〈0.00001）. CONCLUSION Compared with the Labiogingival groove surgical approach, nasal endoscopy approach can significantly shorten the operation time, hospital stay and healing time, and also can reduce the amount of intraoperative bleeding, postoperative recurrence rate and so on. In view of the fact that this study is limited to domestic literature and may have the effect of bias of publication, more longitudinal, cross-sectional and highquality randomized controlled studies at home and abroad are needed.
