摘要
目的在腹腔镜下阑尾炎切除术中比较Hem-o-lok结扎锁和Endoloop套扎处理阑尾根部的手术效果。方法 2017年5月～2018年5月对我院120例成人急性阑尾炎患者实施腹腔镜下阑尾切除术,根据手术方法分为H组和E组,每组60例。H组采用Hem-o-lok结扎锁对阑尾根部处理,E组采用Endoloop套扎阑尾根部,并比较两组患者手术时间、住院时间、手术费用、手术并发症等情况。结果 H组手术时间(42.83±6.52)min,住院时间(4.23±0.78)d,手术费用(4273.6±51.22)元,手术并发症4例(6.66%)。E组手术时间(46.10±7.07)min,住院时间(4.17±0.70)d,手术费用(5137.2±83.17)元,手术并发症6例(10.00%)。两组患者病理类型、住院时间、手术并发症的发生率比较,差异无统计学意义(P>0.05),而E组患者的手术时间、住院的手术费高于H组,差异有统计学意义(P<0.05)。结论 Hem-o-lok结扎锁和Endoloop套扎均可以在腹腔镜阑尾切除术中安全有效的处理阑尾炎根部,但使用Hem-o-lok结扎锁的手术时间更短,手术相关费用更低,具有很好的运用前景。
Objective To compare the surgical outcomes of Hem-o-lok ligation lock and Endoloop ligation in the root of appendixduring laparoscopic appendicitis resection.Methods From May 2017 to May 2018,120 patients with acute appendicitis in our hospitalunderwent laparoscopic appendectomy.According to the surgical method,they were divided into H group and E group,60 in eachgroup.The H group was treated with Hem-o-lok ligation lock for the root of the appendix.The E group was treated with Endoloop forthe root of the appendix.The operation time,hospitalization time,operation cost and surgical complications of the two groups werecompared.Results The operation time of group H was （42.83±6.52）min,the length of hospital stay was （4.23±0.78）d,the operation costwas（4273.6±51.22） yuan,and the operation complications were4 cases（6.66%）.The operation time of group E was（46.10±7.07）min,thelength of hospital stay was （4.17±0.70）d,the operation cost was （5137.2±83.17）yuan,and the operation complications were 6 cases（10.00%）.There was no significant difference in the pathological type,length of hospital stay,and incidence of surgical complicationsbetween the two groups（ P〉0.05）.The operation time and hospitalization cost of patients in group E were higher than those in group H,the difference was statistically significant（ P〈0.05）.Conclusion Hem-o-lok ligation lock and Endoloop ligation are safe and effectivein the treatment of appendicitis root in laparoscopic appendectomy,but the use of Hem-o-lok ligation lock shorter operation time,surgery-related costs lower,has a good application prospects.
