摘要
目的通过对高血压患者住院费用的分析,探索其费用变化趋势及相关因素,为掌握该疾病的诊疗费用水平、控制费用、合理利用卫生资源等提供参考依据。方法采用新灰色关联法和结构变动度分析,定量评价2013—2017年高血压患者住院费用各因素之间的关联程度和结构变动。结果各项费用中药费的关联度最大,其次是检查费、检验费。5年间住院费用的结构变动度为31. 38%,药费的结构变动贡献率最大,其次是检验费和检查费,三者累计贡献率为83. 01%。结论为较好地控制高血压患者住院费用,应进一步规范治疗及检查项目应用规范、加强用药管理措施、实施DRGs费用标准,有效利用医疗卫生资源,降低患者经济负担,进一步验证灰色关联和结构变动度适合医疗费用间关联程度的衡量、动态趋势及结构变化的总体特征。
Objective To explore the tendency of and influencing factors for hospitalization expenses of patients with hypertension through the analysis of their hospital costs, and thrther to provide reference for understanding the cost level, controlling costs and rationally utilizing medical resources. Methods New grey correlation degree analysis method and the structure changes analysis were used to quantitatively evaluate the correlation degree between various factors and structural changes of hospitalization expenses from 2013 to 2017. Results The highest correlation among all the foes was in drug costs, followed by lahoratory fees and inspection fees. The structural change degree of hospitalization expenses over the five years was 31.38 % , with the largest contribution of structure changes in drug costs, followed by laboratory fees and inspection fees. The cumulative contribution rate was 83.01%. Conclusion To better control the hospitalization expenses of hypertension patients, we should further standardize the criteria of treatment and examination, strengthen drug management, implement the DRGs cost standard and utilize health resources more effectively, so as to reduce patients＇ economic burden. Besides, it is verified that the grey correlation analysis and structure change degree are applicable in the measurement of correlation between medical expenses and of the dynamic trends and general characteristics of structural changes.
