摘要
目的探讨多层螺旋CT检查对早期肺癌的诊断价值。方法选取2016年1月至2018年1月就诊的疑似早期肺癌74例,均行胸部X线片检查及多层螺旋CT检查,以病理检查为金标准,分析两种检查手段在早期肺癌中的诊断价值。结果 74例中,经病理检查确诊23例为诊周围型肺癌,23例为中央型肺癌,9例为肺结核,7例为纵隔恶性淋巴瘤,7例为肺部感染,5例为肺部纤维瘤;多层螺旋CT检查的准确度(89. 19%)、灵敏度(91. 30%)、特异度(85. 71%)、阳性预测值(97. 67%)、阴性预测值(85. 71%)均较X线高(67. 57%、71. 74%、60. 71%、75. 00%、56. 67%];多层螺旋CT检查的检查费用(652. 43±53. 67)元,较胸部X线片(110. 62±20. 68)元更高,检查时间(8. 44±1. 21) min较胸部X线片检查(2. 32±0. 81) min长,差异有统计学意义(P <0. 05)。结论与胸部X线片检查比较,多层螺旋CT检查诊断早期肺癌可获得更佳的临床诊断效果,且诊断准确度、灵敏度、特异度均高,可为该病的临床诊治提供可靠的影像学依据。
Objective To analyze the diagnostic value of multi - slice spiral CT in early lung cancer. Methods From January 2016 to January 2018, 74 patients with suspeded early lung cancer were seleded. All patients underwent chest X - ray examination and multi - slice spiral CT. The diagnostic value of the two methods in early lung cancer was analyzed according to the gold standard of pathological examination. Results Among 74 patients suspeded of early lung cancer, 23 were diagnosed as peripheral lung cancer, 23 as central lung cancer, 9 as pulmonary tuberculosis, 7 as mediastinal malignant lymphoma, 7 as pulmonary infedion and 5 as pulmonary fibroma. The accuracy, sensitivity and specificity of multi - slice spiral CT were 89. 19% , 91.30% , respedively. Degree （85.71%）, positive predidive value （97.67%） and negative predidive value （85.71%） were higher than those of the control group [ （67.57%）, （71.74%）, （60.71%）, （75.00%） and （56. 67% ）] and the difference was statistically significant （ P The examination time was （8.44＋1.21） minuteslonger than chest X- ray examination [ （110.62＋20.68） yuan, （2.32＋0.81） minutes], and the difference was statistically significant （P 〈0.05） . Conclusion Compared with chest X - ray examination, multi - slice spiral CT can obtain better clinical diagnosis effect for early lung cancer, and has high diagnostic accuracy, sensitivity and specificity, which can provide reliable imaging basis for clinical diagnosis and treatment of this disease.
