摘要
目的分析肾嗜酸细胞瘤(RO)及后肾腺瘤(MA)的临床及影像资料,探讨肾少见良性腺瘤的影像学特征。方法回顾性分析21例RO及7例MA患者的临床资料,并记录病灶的部位、大小、与正常肾实质夹角、CT平扫及增强强化方式、MRI平扫信号及强化方式、DWI信号特点,测量CT平扫及三期增强中病灶及同侧正常肾皮质的CT值。结果RO病灶平均直径为3.7cm,CT平扫其CT值与周围肾实质相仿,增强后不均匀强化,6例可见中心星状瘢痕、5例可见节段反转强化征、7例病灶可见假包膜;MRI平扫示病灶信号混杂,DWI呈稍高信号且随着b值增加信号减弱,其中1例病灶中心星状瘢痕随着b值增加信号稍增强。MA病灶平均直径为2.9cm,CT平扫其CT值与周围肾实质相仿,增强后不均匀轻-中度强化,部分病灶可见延迟强化;MRI平扫示病灶信号混杂,DWI呈稍高信号且随着b值增加信号增强。结论 RO及MA为肾脏少见良性腺瘤,在影像学上有一定的特征性,术前影像学检查对于临床诊疗有一定的帮助。
Objective To analyze the CT and MR images of renal oncocytoma （RO） and metanephric adenoma （MA）, and tried to figure out the characteristics.
Methods Twenty-one patients with RO and 7 patients with MA were included. The clinical data, location, size, the angle with the surface of normal renal parenchyma, the features of unenhanced and enhanced CT, routine MRI, enhanced MRI and DWI, the CT values of renal mass and homolateral renal cortex of unenhanced and enhanced CT were analyzed.Results The average diameter of RO was 3.7 cm, and the CT values of lesions were similar to homolateral renal parenchyma, but heterogeneously enhanced after enhanced CT and MRI, of which 6 cases with central stellate scar, 5 cases with segmental enhancement inversion, 7 cases with pseudocapsule. The lesions were heterogeneous hypointense on T 1 WI and heterogeneous hyperintense on T 2 WI, and mild hyperintense on DWI, slightly decreased as the b value increased, but intensity of the central stellate scar was increased as b value increased. The average diameter of MA was 2.9 cm, and the CT values of lesions were similar to homolateral renal parenchyma, but heterogeneously mildly or moderately enhanced after enhanced CT and MRI, and several lesions with delayed enhancement. The lesions were heterogeneous in routine MRI, and the parenchyma was mild heterogeneous hyperintense on DWI, increased as the b value increased.Conclusions RO and MA were rare primary renal adenomas, with several CT and MRI characteristics, which can help to improve the clinical diagnosis and treatment plan before surgery.
