摘要
目的对比分析昭通市第一人民医院常用的两种抗抑郁药盐酸文拉法辛胶囊(博乐欣)和氟哌噻吨美利曲辛片(黛力新)的对老年抑郁症的治疗效果及治疗成本,以便更合理的指导临床用药。方法采用随机双盲前瞻性病例对照研究的方法,随机抽取昭通市第一人民医院2017年10月—2018年4月就诊的老年抑郁症状服用这两种药物患者100例进行研究,根据疗效制作成本效果数据表格,分析两种药物之间的成本效果。结果博乐欣和黛力新在综合医院治疗老年抑郁症状疗效相当,但治疗成本,黛力新组费用明显低于博乐欣组:治疗轻中度患者1个疗程至无症状或症状减轻(χ~2=0.5,1.0,P>0.05),而费用黛力新需(101.50±40.65)元,博乐欣需(261.45±193.96)元(t=3.998,P<0.05),治疗中度及重度患者1个疗程至症状减轻或无效(P>0.05),费用方面黛力新组需(203.00±40.65)元,博乐欣需(784.35±193.96)元(t=7.12,P<0.05),且黛力新一天1次或者2次,老年患者容易记忆,博乐欣每日需服药3次,并且需根据医嘱增减服药量,老年患者不易记忆,使用黛力新治疗抑郁症,减轻老年人经济负担的同时,患者依从性更高,更趋合理,维持治疗后更节省费用。结论治疗老年抑郁症状,黛力新费用更低,依从性更好,更能减轻老年患者的经济负担和提高患者的依从性,更适合老年患者,可作为优先推荐使用。
Objective To compare and analyze the effect and cost of venlafaxine hydrochloride capsules（venlafaxine）and flupentixol and melitracen tablets（deanxit） in treatment of senile depression in the first people＇s hospital of Zhaotong in order to guide the clinical medication more rationally. Methods 100 cases of senile depression patients diagnosed in the first people＇s hospital of Zhaotong from October 2017 to April 2018 were randomly selected, and the data excel of cost effect was made according to the curative effect, and the cost effect of the two drugs was analyzed. Results The curative effect of the two drugs was similar, but the treatment cost in the deanxit group was obviously lower than that in the venlafaxine group, after one treatment course, the symptoms of the patients in the mild and moderate degree disappeared or the symptoms were relieved（P〉0.05）, but the cost of deanxit and venlafaxine needed the（101.50±40.65）yuan and from（261.45 ±193.96）yuan（t =3.998, P 〈0.05）, after one treatment course, the symptoms of patients in the moderate and severe degrees were relieved or ineffective（P〉0.05）, and the cost in the deanxit group and in the venlafaxine group needed（203.00 ±40.65） yuan and（784.35±193.96）yuan（t =7.12, P〈0.05）, and the senile patients were easy to remember by taking deanxit orally once or twice, and venlafaxine three times, and the deanxit in treatment of depressions could relieve the economic burdens of senile patients and save more costs with higher compliance. Conclusion The cost of deanxit in treatment of senile depression is lower with better compliance, which can better relive the economic burdens and improve the compliance of senile patients, and it is more suitable for the senile patients, and it can be recommended for preference.
