评价核磁共振成像技术诊断膝关节前交叉韧带断裂的临床准确性 被引量:8

Evaluation on Clinical Accuracy of MRI in Diagnosis of Anterior Cruciate Ligament Injury of Knee Joint

摘要 目的评价评价核磁共振成像技术诊断膝关节前交叉韧带断裂的效用,总结诊断经验。方法 2013年1月—2017年4月,医院经膝关节镜诊断确诊并在术前采用MRI诊断怀疑伴膝关节前交叉韧带损伤184例,回顾性分析。结果 MRI对韧带损伤的灵敏度94.35%(117/124)、特异度76.67%(46/60)、阳性预测值89.31%(117/131)、阴性预测值86.79%(46/53)、符合率88.59%(163/184)。前交叉韧带损伤对象M、L分别为(29.04±0.62)、(30.65±1.00)高于无损伤对象(27.56±0.52)、(28.69±0.63),有损伤对象NW、NWI分别为(17.92±2.21)、(0.23±0.01)低于无损伤对象(20.25±1.53)、(0.27±0.01),差异有统计学意义(t=4.22、8.52、5.38、3.86,P=0.000、0.000、0.000、0.001<0.05)。结论核磁共振成像技术诊断膝关节前交叉韧带断裂效用尚可。 Objective To evaluate the effect of MRI in diagnosis of anterior cruciate ligament injury of knee joint and summarize the diagnosis experience. Methods 184 cases of patients suspected with anterior cruciate ligament injury of knee joint in our hospital from January 2013 to April 2017 were selected, and diagnosed with MRI before surgery, and the results were retrospectively analyzed. Results The sensitivity, specificity, positive predicative value, negative predicative value, coincidence rate of MRI to the ligament injury were respectively 94.35%(117/124), 76.67%(46/60),89.31%(117/131), 86.79%(46/53), 88.59%(163/184), and the M and L of objects with anterior cruciate ligament were higher than those without injuries, [(29.04±0.62),(30.65±1.00) vs(27.56±0.52),(28.69±0.63)], and the NW and NWI of objects with injures were lower than those without injuries, [(17.92±2.21),(0.23±0.01) vs(20.25±1.53),(0.27±0.01) ],and the differences were statistically significant(t=4.22, 8.52, 5.38, 3.86, P=0.000, 0.000, 0.000, 0.001 〈0.05). Conclusion The effect of MRI in diagnosis of anterior cruciate ligament injury of knee joint is fair.
