摘要
目的探讨内镜下高频电凝刀电切除术治疗结肠息肉的临床疗效与安全性。方法选取2014年2月—2017年2月该院276例结肠息肉患者为研究对象,根据硬币投掷法将受试者分为对照组和研究组,每组138例,对照组患者给予传统的开腹手术治疗,研究组患者给予内镜下高频电凝切除术治疗,比较两组患者手术时间、术中出血量、卧床时间、住院时间、日常生活能力评分、治愈率及并发症发生率,随访1年后,比较两组患者的复发情况。结果研究组患者的手术时间、术中出血量、卧床时间、住院时间及ADL评分均显著少于对照组,两组比较差异有统计学意义(t手术时间=11.62,t术中出血量=35.42,t卧床时间=8.45,t住院时间=6.73,t ADL评分=9.56;P<0.05),研究组患者术后并发症发生率及复发率分别为5.07%、1.45%,显著低于对照组的23.19%、10.14%,两组比较差异有统计学意义(χ~2并发症=15.83,χ~2复发率=11.65;P<0.05),两组患者治愈率比较差异无统计学意义(χ~2=1.63,P>0.05)。结论内镜下高频电凝切除术可有效缩短手术时间及患者术后康复时间,减少术中出血量、术后并发症发生率及复发率,提高患者日常生活能力,在结肠息肉治疗中具有重要应用。
Objective To study the clinical curative effect and safety of endosocopic high frequency electrocoagulation knife excision in treatment of colonic polyp. Methods 276 cases of patients with colonic polyp admitted and treated in our hospital from February 2014 to February 2017 were selected and divided into two groups with 138 cases in each,the control group used the traditional laparotomy, while the research group were treated with endoscopic high frequency electrocoagulation, and the operation time, intraoperative bleeding amount, time in bed, length of stay, ADL, curing rate and incidence rate of complications were compared between the two groups, after 1-year follow-up, the recurrence was compared between the two groups. Results The operation time, intraoperative bleeding amount, time in bed, length of stay and ADL score in the research group were obviously lower than those in the control group, and the differences between groups were obvious（t operation time =11.62, t intraoperative bleeding amount =35.42, t time in bed =8.45, t=length of stay =6.73, t=ADL score=9.56; P 〈0.05）, and the incidence rate of complications after surgery and recurrence rate in the research group were obviously lower than those in the control group（5.07%, 1.45%vs 23.19%, 10.14%）, and the differences between groups were obvious（ χ^2 complication=15.83, χ^2 recurrence rate=11.65; P 〈0.05）, and the difference in the cure rate between the two groups was not statistically significant（χ^2=1.63, P〉0.05）. Conclusion The endosocopic high frequency electrocoagulation knife excision can effectively shorten the operation time and postoperative recovery time, reduce the intraoperative bleeding amount, incidence rate of postoperative complications and recurrence rate, and improve the ADL of patients, and it is of important significance in treatment of colonic polyps.
