摘要
目的探究不同通量透析器对维持性血液透析患者残存肾功能的影响。方法选取该院2016年8月—2017年8月收治的204例接受维持性血液透析治疗患者,并按照双盲法将其分为两组,观察组与对照组分别予以高通量、低通量透析器治疗,并对两组患者治疗前与治疗后6个月残余肾功能、肌酐、尿素氮、β_2-微球蛋白与白蛋白水平变化与SF-36量表评分进行对比。结果观察组患者治疗后残余肾功能(1.20±0.12)mL/min高于对照组的(0.38±0.06)mL/min(t=61.727 4,P=0.000 0);观察组治疗后β_2-微球蛋白与白蛋白水平(14.48±3.97)mg/L、(38.00±1.01)g/L,优于对照组的(19.45±2.95)mg/L、(34.97±2.26)g/L(t=10.148 4,P=0.000 0;t=12.362 1,P=0.000 0);观察组患者治疗后生理功能、生理职能、躯体疼痛、总体健康、情感角色、心理健康评分(31.00±2.97)分、(4.97±0.80)分、(8.67±1.40)分、(11.97±0.26)分、(4.97±1.26)分、(25.09±2.96)分,高于对照组的(26.09±3.00)分、(4.41±0.70)分、(9.29±1.00)分、(10.25±2.09)分、(4.51±0.97)分、(22.07±3.09)分(t=11.746 7,P=0.000 0;t=5.3205,P=0.000 0;t=6.343 4,P=0.000 0;t=8.248 0,P=0.000 0;t=62.921 6,P=0.003 9;t=7.128 0,P=0.000 0)。结论高通量透析器在维持性血液透析患者中,可保护残余肾功能,提高患者的生活质量。
Objective To study the effect of different flux dialyzers on the residual renal functions of patients with maintenance hemodialysis. Methods 204 cases of patients with maintenance hemodialysis admitted and treated in our hospital from August 2016 to August 2017 were selected and randomly divided into two groups, the observation group and the control group were respectively treated with high flux and low flux dialyzers, and the residual renal function,creatinine, urea nitrogen, and beta 2-microglobulin, changes of albumin level and SF-36 scale score in 6 months before and after treatment were compared between the two groups. Results The residual renal function in the observation group was higher than that in the control group, [(1.20±0.12) mL/min vs(0.38±0.06) mL/min(t=61.727 4, P=0.000 0)],and the Beta 2-microglobulin and albumin levels in the observation group after treatment were better than those in the control group, [(14.48±3.97) mg/L,(38.00±1.01)g/L vs(19.45±2.95)mg/L,(34.97±2.26) g/L(t=10.148 4, P =0.000 0; t =12.362 1, P =0.000 0)], and the physical function, mental function, somatic pain, total health, emotion role, mental health scores in the observation group were higher than those in the control group,[(31.00±2.97)points,(4.97±0.80)points,(8.67±1.40)points,(11.97±0.26)points,(4.97±1.26)points,(25.09±2.96)points vs(26.09±3.00)points,(4.41±0.70)points,(9.29 ±1.00)points,(10.25 ±2.09)points,(4.51 ±0.97)points,(22.07 ±3.09)points(t =11.746 7, P =0.000 0; t =5.320 5, P =0.000 0; t=6.3434, P=0.000 0; t=8.248 0, P=0.000 0; t=62.9216, P=0.003 9; t=7.128 0, P=0.000 0)]. Conclusion The high flux dialyzer in patients with maintenance hemodialysis can protect the residual renal function and improve the quality of life.
