摘要
目的观察血清钙调蛋白(CAM)、低氧诱导因子(HIF)-1α和髓鞘碱性蛋白(MBP)在急性脑梗死患者检测中的临床意义。方法选择就诊的急性脑梗死患者108例,为急性脑梗死组。选择同期行健康体检者35例,为健康对照组。采用酶联免疫吸附实验方法检测各组血清CAM、HIF-1α和MBP水平。观察急性脑梗死组和健康对照组血清CAM、HIF-1α和MBP水平的变化,急性脑梗死患者血清CAM、HIF-1α和MBP水平与脑梗死分型,梗死面积和预后的关系及其指标间的相互关系。结果急性脑梗死组的血清CAM、HIF-1α和MBP水平明显高于健康对照组(P<0.01)。急性脑梗死血清CAM、HIF-1α,MBP水平在TACI,PACI,POCI和TACI分型中出现依次降低(P<0.01)。急性脑梗死患者血清CAM、HIF-1α,MBP水平随着脑梗死面积的增加或者预后恶化,其水平出现明显升高(P<0.01)。急性脑梗死患者血清CAM水平与HIF-1α(r=0.7167,P<0.01)和MBP(r=0.837,P<0.01)水平呈正相关,而HIF-1α水平与MBP水平呈正相关(r=0.684,P<0.01)。结论 CAM、HIF-1α和MBP水平参与了急性脑梗死的疾病的发生发展过程,对于疾病的严重程度和预后评估具有重要临床价值。
Objective To investigate the clinical significance of serum calmodulin( CAM),hypoxia inducible factor( HIF) 1α and serum myelin basic protein( MBP) in the detection of acute cerebral infarction. Methods A total of 108 patients with acute cerebral infarction who were treated in our hospital from January 2015 to June 2017 were enrolled as acute cerebral infarction group,at the same time,the other 35 healthy subjects who received physical examination in our hospital were enrolled as healthy control group. The serum levels of CAM,HIF-1α and MBP were measured by enzyme linked immunosorbent assay. The changes of serum levels of CAM,HIF-1α and MBP were observed and compared between the two groups. Moreover the correlation between the serum levels of CAM,HIF-1α,MBP and the classification of acute cerebral infarction,infarct size and prognosis was analyzed,moreover,the correlation among indexes was also analyzed. Results The serum levels of CAM,HIF-1α and MBP in acute cerebral infarction group were significantly higher than those in healthy control group( P〈0. 01). The serum levels of CAM,HIF-1α and MBP in acute cerebral infarction were decreased in order of TACI,PACI,POCI,TACI types( P〈0. 01). The serum levels of CAM,HIF-1α and MBP in acute cerebral infarction group significantly increased along with the increasing of cerebral infarct size or prognosis deterioration( P〈0. 01). The serum levels of CAM in acute cerebral infarction group were positively correlated with those of HIF-1α( r = 0. 7167,P〈0. 01) as well as MBP( r = 0. 837,P〈0. 01),meanwhile the serum levels of HIF-1α were positively correlated with those MBP( r =0. 684,P〈0. 01). Conclusion The CAM,HIF-1α and MBP are involved in the occurrence and development of acute cerebral infarction and have important clinical value in the evalution of severity degrees and prognosis of the disease.
