抗环瓜氨酸肽抗体检测对类风湿性关节炎的临床价值 被引量:1

摘要 目的探讨抗环瓜氨酸肽(CCP)抗体和类风湿因子(RF)检测对类风湿性关节炎(RA)的临床价值。方法选择184例RA患者(RA组)和78例其他自身免疫性疾病患者(非RA组)为研究对象,检测患者血清中抗CCP抗体和RF水平,分析其在RA中的诊断价值。结果抗CCP抗体检测的特异度显著高于RF检测(P<0.05);抗CCP抗体和RF联合检测的灵敏度高于抗CCP抗体和RF单独检测;活动组的抗CCP抗体及RF滴度均显著高于缓解组(P<0.05),且临床表现明显比缓解组严重。结论抗CCP抗体在RA患者诊断中有较好的灵敏度和特异度,与RF联合检测可提高RA诊断的灵敏度和特异度。抗CCP抗体的滴度水平与疾病活动度呈正相关,可为临床对RA的早期诊断、评估病情及预后判断提供依据。 Objective To explore the clinical value of anti-cyclic citrullinated peptide(CCP) antibody and rheumatoid factor(RF) in the diagnosis of rheumatoid arthritis(RA). Methods A total of 184 RA patients(RA group) and 78 other autoimmune disease patients(non-RA group) were selected as study object. The anti-CCP antibody and RF levels in the patient's serum were tested and their diagnostic values in RA were analyzed. Results The specificity of anti-CCP antibody detection was significantly higher than that of RF detection(P〈0.05). The sensitivity of anti-CCP antibody and RF combined detection was higher than that of anti-CCP antibody and RF alone. The anti-CCP antibody and RF titer of the active group were significantly higher than those of the remission group(P〈0.05), and the clinical manifestations were significantly more severe than that in the remission group. Conclusion Anti-CCP antibody has good sensitivity and specificity in the diagnosis of RA patients. Combined with RF detection can improve the sensitivity and specificity of RA diagnosis. The titer level of anti-CCP antibody is positively correlated with disease activity, which can provide a basis for clinical diagnosis, assessment of disease and prognosis of RA.
