神经生长因子对新生儿缺氧缺血性脑病患儿神经行为发育及预后的影响 被引量:3

Effect of nerve growth factor on neurobehavioral development and prognosis in neonatal hypoxic-ischemic encephalopathy

摘要 目的:探讨神经生长因子辅助治疗新生儿缺氧缺血性脑病临床效果。方法:选取某院收治的新生儿缺氧缺血性脑病患儿104例,利用随机数表法分为2组,每组各52例。对照组采取胞磷胆碱与脑蛋白水解物进行治疗,观察组在此基础上加用神经生长因子进行治疗,比较2组患儿临床疗效、神经行为发育指标(NPY,NSE,MBP)、血清肌酸激酶(CK)水平、脑型同工酶水平(CK-BB)、肌酸激酶同工酶(CK-MB)、神经行为功能评分及神经系统后遗症。结果:2组患儿治疗总有效率比较,观察组(94.23%)较对照组(80.77%)高,差异有统计学意义(P<0.05);治疗后,2组患儿NPY,NSE,MBP,CK,CK-MB,CK-BB水平均显著降低,且观察组较对照组低,差异有统计学意义(P<0.05);治疗第7天、第14天,2组患儿NBFS评分均明显提高,且观察组优于对照组,差异有统计学意义(P<0.05);2组后遗症发生率比较,观察组(7.69%)较对照组(23.08%)低,2组比较差异有统计学意义(P<0.05)。结论:神经生长因子联合胞鳞胆碱及脑蛋白水解物治疗,可有效改善缺氧缺血性脑病患儿神经行为功能,降低神经系统后遗症发生率。 Objective :To investigate the clinical effect of nerve growth factor (NGF) in treatment of neonatal hypoxic-ischemic en- cephalopathy ( HIE ). Methods :104 HIE children were randomly divided into two groups, 52 cases in each. The control group was giv- en citieoline and cerebroprotein hydrolysate, while the observation group was treated with NGF based on the regime of the control group. The clinical efficacy, neurobehavioral development indexes ( NPY, NSE, MBP ), serum creatine kinase ( CK ), creatine kinase (CK-BB) ,creatine kinase isoenzyme ( CK-MB), neurobehavioral function scores,and neurological sequelae were compared between the two groups. Results :The total effective rate of the observation group (94.23%) was higher than that of the control group (80.77% ,P 〈 0.05 ). After the treatment, the levels of NPY, NSE, MBP, CK, CK-MB, and CK-BB in the two groups deereased, lower in the ob- se^wation group than in the control group ( P 〈 0.05 ). 7d and 14d after the treatment, the neurobehavioral functional scale ( NBFS ) of the two groups increased, with the observation groupg scale being higher than the control groupg (P 〈 0.05 ). The incidence of sequelae in the observation group (7.69%) was lower than that in the control group ( 23.08%, P 〈 0.05 ). Conclusion : NGF combined with citicoline and cerebroprotein hydrolysate in treatment of HIE is effective. It can improve neurobehavioral function and deerease the inci- dence of neurological sequelae.
