摘要
【目的】了解我院2013至2016年门诊心血管西药的使用情况及变化趋势,分析临床用药的合理性。【方法】对2013至2016年门诊心血管西药的种类、费用、用药频度(defined daily doses,DDDs)法等进行统计分析。【结果】除强心药以外,2013至2016年门诊心血管类西药费用与DDDs逐年上升,钙通道阻滞药、血管紧张素受体拮抗药(angiotensin receptor blocker,ARB)与血脂调节药的费用和DDDs稳居前3名,4年间血管紧张素转化酶抑制药(angiotensin converting enzyme inhibitors,ACEI)和ARB类药物的费用和DDDs均持续上升,但ACEI类的费用与DDDs明显低于ARB类药物;2013至2016年药品费用金额与DDDs排名前3的西药没有明显变化。药品费用前10名的西药日用药金额均小于10元。【结论】我院临床医师倾向于使用国内外指南推荐的一线西药,患者用药负担较轻。临床医师更倾向将ARB类药物作为临床首选,而没有选择循证医学证据更充分的ACEI类药物,ARB类药物在临床应用上还存在不合理性。
【Objective】To evaluate the utilization status and tendency of cardiovascular drugs in the dispensary for outpatients of our hospital from 2013 to 2016, and analyze the rationality of clinical medication. 【Methods】The data of cardiovascular drugs for outpatients from 2013 to 2016 were statistically analyzed, including the variety, drug cost, defined daily doses（DDDs）, etc.【Results】The drug cost and DDDs of cardiovascular drugs for outpatients rose year by year from 2013 to 2016 except cardiotonic drug. The top 3 of drug cost and DDDs were CCB, angiotensin receptor blocker（ARB） and antilipemic agents. The drug cost and DDDs of angiotensin converting enzyme inhibitors（ACEI） and ARB increased continuously during the four years, but the drug cost and DDDs of ACEI were significantly lower than those of ARB. The top 3 drugs both in the drug cost and DDDs showed no significant change from 2013 to 2016.The daily drug cost was less than 10 yuan in the top 10 drugs.【Conclusion】The doctors in our hospital tend to use the first-line drugs recommended by the domestic and foreign guidelines. The patients＇ economic burden was lighter. ARB, but not ACEI with more evidence-based medical evidence, is selected as the first choice by clinical doctors. There is still irrationality in clinical application of ARB drugs.
