摘要
目的探讨胃肠道间质瘤增强CT特征性表现及其与危险度分级的相关性。方法回顾性收集经本院病理确诊为胃肠道间质瘤的患者47例,将CT图像表现与病理危险度分级间进行卡方检验或Fisher精确概率检验,另外将三种不同方法测量的间质瘤大小—横断面长径、三维长径和三维体积与危险度分级间分别行差异性检验和Spearman相关性分析,用ROC曲线分别评价三个指标区分极低/低危险度间质瘤的最佳阈值及诊断效能并进行对比。结果肿瘤密度、轮廓及静脉期强化程度与肿瘤危险度间有统计学意义。横断面长径、三维长径和三维体积与危险度分级间均存在显著相关性(P<0.001,r=0.504、0.516、0.522);均能够区分极低/低危险度组间质瘤,曲线下面积分别为0.790、0.797、0.806。结论胃肠道间质瘤具有一定特征性CT表现,其中肿瘤大小、密度、轮廓及静脉期强化程度可以判断间质瘤危险度,尤其是在大小方面,肿瘤体积值可以更好的区分间质瘤危险度。
Objective To investigate the relationship between contrast-enhanced CT features and the malignant risks of Gastrointestinal Stromal Tumors(GISTs). Methods Forty-seven patients with GISTs confirmed by pathology were retrospectively studied. The CT features were compared with different risks of pathologic findings by chi-square test or Fisher exact test. The correlation between the size of GISTs measured by three methods(major axis in cross-section,major axis in three-dimensional,volume in three-dimensional) and different malignant risks were analyzed by Spearman correlation analysis. The diagnostic performances of these three indexes for distinguishing different malignant risks were assessed using the area under the receiver operating characteristic curve(AUC). Results Density,contour and degree of enhancement in venous phase among different risk groups were statistically different. The size of GISTs measured by three methods was correlated significantly with malignant risks(all P 0. 001; r = 0. 504,0. 516,0. 522,respectively). They can also differentiate very low/low malignant risk from other risks(all P〈0. 05,AUC = 0. 790,0. 797,0. 806,respectively). Conclusion GISTs have characteristic CT features. The size,Density,contour and degree of enhancement in venous phase may evaluate the malignant risks of GISTs,especially the volume of tumer size.
