摘要
目的:研究分析风湿性心脏病在同期实施二尖瓣、主动脉瓣、三尖瓣置换的临床治疗效果。方法:收治风湿性心脏病患者50例,患者在同期实施二尖瓣、主动脉瓣、三尖瓣置换术。结果:患者住院治疗期间,死亡1例;术后早期出现低心排综合征7例,对其积极治疗后得到了有效的纠正;术后患者发生肾功能衰竭死亡1例;术后2次开胸止血4例;术后伤口感染4例;肺部感染5例;经过头颅CT检查发现1例患者有少量的蛛网膜下腔出血现象;1例发生Ⅲ度房室传导阻滞现象。患者在术后进行随访,其中心功能恢复至Ⅰ级18例,恢复至Ⅱ级23例,失访3例。结论:对风湿性心脏病患者治疗时应该将二尖瓣、主动脉瓣、三尖瓣置换术同期实施,可以有效帮助患者恢复右心功能,改善心脏血流动力学的特性,使患者的心功能得到有效的改善,效果良好。
Objective：To study and analyze the clinical effect of mitral valve,aortic valve and tricuspid valve replacement for rheumatic heart disease during the same period.Methods：50 cases of rheumatic heart disease were selected.They were treated with mitral valve,aortic valve and tricuspid valve replacement during the same period.Results：During the period of hospitalization,1 case died,and 7 cases of low cardiac syndrome were presented early after the operation,and effective correction was achieved after active treatment.After operation,1 case died of renal failure,4 cases were hemostasis after second thoracotomy,4 cases of postoperative wound infection and 5 cases of pulmonary infection.A small amount of subarachnoid hemorrhage was found in 1 patient after the head CT examination.1 case of atrioventricular block was observed.The patients were followed up after operation,of which 18 cases of cardiac function recovered to grade Ⅰ,23 cases recovered to grade Ⅱ,and 3 cases were lost.Conclusion：Mitral valve,aortic valve and tricuspid valve replacement should be performed in patients with rheumatic heart disease at the same time.It can effectively help patients recover right heart function and improve the characteristics of cardiac hemodynamics.The heart function has been effectively improved with good results.
