摘要
目的探讨甲状腺结节良恶性质应用CT与MRI诊断鉴别的价值,以便为临床诊治提供参考依据。方法选择我院2013年4月~2017年4月收治的甲状腺结节患者210例作为研究对象,入选对象有完整临床资料,共计病变结节240个,均经术后病理穿刺活检证实为甲状腺结节,同时术前均接受常规CT与MRI诊断,分别定义为CT组与MRI组。对两组影像学特点进行总结,并比较两组诊断良性与恶性甲状腺结节的灵敏性、特异性、阳性/阴性预测值、准确率。结果术后病理活检诊断甲状腺结节恶性64个、良性176个,其中CT诊断甲状腺结节良性174个、恶性66个,MRI诊断甲状腺结节良性186个、恶性54个;对灵敏性、特异性、阳性/阴性预测值比较可以看出,MRI组与CT组在灵敏性、阳性预测值比较差异有统计学意义(P<0.05),而特异性、阴性预测值、准确率比较无差异(P>0.05)。结论将甲状腺结节良恶性质鉴别诊断中应用CT与MRI诊断都有不错的价值,诊断准确率高,且各有优势,临床诊断中若能联合应用可进一步提高诊断符合率。
Objective To investigate the value of CT and MRI in the differential diagnosis of benign and malignant thyroid nodules, so as to provide reference for clinical diagnosis and treatment of those disorders.Methods 210 patients with thyroid nodules, who were referred to our hospital from April 2013 to April2017, with complete clinical data, were selected as the research subjects. In this cohort, a total of 240 lesions were all the thyroid nodule as confirmed by postoperative pathological biopsy. Meanwhile, all patients underwent preoperative conventional CT and MRI and were defined as CT group and MRI group. Then, the imaging characteristics of two groups were summarized, and the sensitivity, specificity, positive or negative predictive value as well as accuracy rate of benign and malignant thyroid nodules were compared between the two groups.Results The postoperative pathology diagnosed that 64 thyroid nodules were malignant and 176 were benign, from which CT diagnosed 174 benig thyroid nodules and 66 malignant ones, while, MRI diagnosed 186 benign thyroid nodules and 54 malignant nodules. In comparison to the sensitivity, specificity, positive or negative predictive value, the difference was statistically significant ( P 〈0.05) between MRI group and CT group in sensitivity and the positive predictive value, but, the specificity, negative predictive value, and accuracy rate had no difference ( P 〉0.05).Conclusion The application of CT and MRI has good value in differential diagnosis of benign and malignant thyroid nodules as both with high diagnostic accuracy. CT or MRI each has advantages in clinical diagnosis. The combined application of both can further improve the diagnosis rate.
