摘要
目的探讨3D腹腔镜与开腹Glisson蒂横断式肝切除术治疗肝癌对机体细胞免疫功能的影响。方法将2014年1月至2017年12月60例肝癌患者,随机分为3D腹腔镜组(3D-LH)和开腹组(OH)各30例进行Glisson蒂横断式肝切除术,主要比较两组患者术前及术后第1、7天细胞免疫指标CD4^+T,CD8^+T细胞水平,分析两组患者细胞免疫功能变化。结果术后1 d,3D-LH组的CD4^+T、CD8^+T细胞免疫指标下降少于OH组;术后7 d,3D-LH组的CD4^+T、CD8^+T恢复快于OH组,组间比较差异均具有统计学意义(P<0.05)。结论相对于开腹肝切除术,3D腹腔镜Glisson蒂横断式肝切除术对肝癌患者机体免疫影响小,恢复更快。
ObjectiveTo compare the effects of 3D laparoscopic hepatectomy(3D-LH)and open hepatectomy(OH)in glissonian access for hepatocellular carcinoma(HCC)on postoperative immunosuppression by analyzing cellular immune function.Methods During January 2014 to December 2017,sixty patients randomly scheduled to undergo hepatectomy were divided into 3D-LH group and OH group,with 30 patients in each group.All patients′peripheral blood was sampled to measure cell-mediated-immunologic markers(CD4^+,CD8^+T cell)before operation and in the mornings of the 1^st,7^th day after surgery.The effects of different operative methods on cellular immune function in patients organism were investigated.ResultsThe decreased level in 3D-LH group was less than that in OH group in postoperative 1^st day.The improvement in the 3D-LH group was more faster than that in the OH group on postoperative 7^th day.There was statistical significance between the two groups(P〈0.05),respectively.ConclusionWhen compared to open hepatectomy,3D laparoscopy liver resection in glissonian approach for HCC,less effects in cellular immune function.
