摘要
妊娠期急性脂肪肝(AFLP)是涉及多个医学领域的危重症疾病,其发病率较低,但起病急、进展快、病情凶险,母婴病死率较高。AFLP的发病由多因素共同作用所致,其临床表现缺乏特异性,应结合实验室检查尽快做出早期识别和诊断,以减少严重并发症和改善预后。AFLP一旦确诊或高度怀疑,均应及时终止妊娠,同时予以多学科的综合支持治疗,严重时需行人工肝治疗,甚至肝移植,以达到较好的预后。未来,基因检测技术的发展将为AFLP的早期诊治提供希望。
Acute fatty liver of pregnancy（AFLP） is a critical illness involving many medical fields.Its incidence is low,but it is acute,rapid,and dangerous.The mortality of mother and child is high.The occurrence of AFLP is caused by multiple factors.Its clinical manifestations are lack of specificity.Early recognition and diagnosis should be made as soon as possible with laboratory tests in order to reduce severe complications and improve the prognosis.Once AFLP is confirmed or highly suspected,the pregnancy should be terminated in time and the multidisciplinary integrated support should be given.In serious cases,the treatment of the liver and even the liver transplantation should be done to achieve a better prognosis.In the future,the development of gene detection technology will provide hope for the early diagnosis and treatment of AFLP.
