摘要
目的观察肩关节镜手术中使用增温灌洗液对患者核心体温与炎性细胞因子水平的影响。方法纳入自2012-11—2014-05完成66例肩关节镜手术,根据随机数表法分为2组(每组33例),观察组灌洗液温度为恒温36℃,对照组灌洗液温度为手术室室温23℃。比较2组低体温、寒颤发生比例,术后6、12、24 h血清中IL-1、IL-6、IL-10含量,以及术后3、6 h引流液中TNF-α、IL-1、IL-6、IL-10含量。结果观察组出现低体温及寒颤的比例明显低于对照组,差异有统计学意义(P<0.05)。观察组术后6、12、24 h血清中IL-6含量明显低于对照组,差异有统计学意义(P<0.05);但2组术后6、12、24 h血清中IL-1、IL-10含量比较差异无统计学意义(P>0.05)。观察组术后3、6 h引流液中TNF-α、IL-1、IL-6、IL-10含量明显低于对照组,差异有统计学意义(P<0.05)。结论肩关节镜手术中使用增温灌洗液可降低患者低体温、寒颤的发生率,有效减轻了局部炎症反应,在一定程度上减少了手术创伤。
Objective To observe the effect of warm lavage solution on core body temperature and inflammatory cytokines level in patients with arthroscopic shoulder surgery. Methods From November 2012 to May 2014, 66 cases of arthroscopic shoulder surgery were performed, and were divided into 2 groups(33 cases in each group) according to the random number table method. The observation group had lavage fluid temperature at a constant temperature of 36℃ and the control group had lavage fluid temperature at the operating room temperature 23℃. The proportions of hypothermia and chills were compared between the two groups. The levels of IL-1, IL-6 and IL-10 in the serum at 6, 12 and 24 hours after operation,and TNF-α, IL-1, IL-6, IL-10 in the drainage fluid at 3 and 6 h after operation were compared. Results The proportion of hypothermia and chills in the observation group was significantly lower than that in the control group, and the difference was statistically significant(P〈0.05). The serum level of IL-6 in the observation group at 6, 12, 24 h after operation was significantly lower than that in the control group(P〈0.05). However, there were no significant differences between the two groups in the level of IL-1, IL-10 in serum at 6, 12 and 24 h after operation(P〈0.05). The content of TNF-α, IL-1, IL-6 and IL-10 in the drainage fluid of the observation group was significantly lower than that of the control group at 3 and 6 hours after operation(P〉0.05). Conclusion The warming lavage fluid in arthroscopic shoulder surgery can reduce the incidence of hypothermia and chills in patients. It effectively alleviates local inflammatory reaction and reduces surgical trauma to a certain extent.
