摘要
目的探讨慢性乙型肝炎伴肝源性糖尿病和肝硬化伴肝源性糖尿病患者肝纤维化指标与胰岛素抵抗的相关性。方法前瞻性研究选自京佑安医院于2015年6月至2017年6月期间收治的慢性乙型肝炎伴肝源性糖尿病(A组)73例与肝硬化伴肝源性糖尿病(B组)患者61例;另选同期收治的慢性乙型肝炎(C组)患者60例与肝硬化(D组)患者50例作为对照组。采用放射免疫法测定肝纤维四项指标。结果 A组和B组血清IV型胶原、Ⅲ型前胶原(PCⅢ)、层黏连蛋白(LN)和透明质酸(HA)含量明显高于C组和D组,且B组高于A组,D组高于C组,均有统计学差异(P<0.05)。A组和B组胰岛素抵抗指数明显高于C组和D组,且B组高于A组,D组高于C组,均有统计学差异(P<0.05)。A组和B组患者Ⅳ型胶原、PCⅢ、LN和HA与胰岛素抵抗指数均呈正相关关系。结论慢性乙型肝炎伴肝源性糖尿病与肝硬化伴肝源性糖尿病患者肝纤维化四项指标与胰岛素抵抗呈线性正相关。
Objective To analyze the correlation of liver fibrosis index and insulin resistance in patients with chronic hepatitis B with hepatic diabetes and liver cirrhosis with hepatic diabetes. Methods 73 cases of chronic hepatitis B with hepatic diabetes（ A group）,61 cases of liver cirrhosis with hepatic diabetes（ B group） 61 case from June 2015 June 2017 were selected,and 60 cases of chronic hepatitis B（ C group）,50 cases of patients with liver cirrhosis（ group D） were selected as the control group. The four indexes of the liver fiber were measured by radioimmunoassay. Results The levels of serum IV collagen,PC Ⅲ,LN and HA in A group and B group were significantly higher than those in C group and D group,and those in B group were higher than those in A group,and those in D group were higher than those in C group. The insulin resistance index in A group and B group was significantly higher than that in group C and group D,and that in B group was higher than that in A group,and that in D group was higher than that in C group（ P〈0. 05）. In chronic hepatitis B with hepatic diabetes,type IV collagen,PC Ⅲ,LN and HA were linearly and positively correlated with the insulin resistance index. In A group and B group,type IV collagen,PC Ⅲ,LN and HA were positively correlated with the insulin resistance index. Conclusion There is a linear positive correlation between the four indexes of liver fiber and insulin resistance in patients with chronic hepatitis B with hepatic diabetes and liver cirrhosis with hepatic diabetes.
