摘要
目的比较改良支架成形术(SP)与经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折的近期临床疗效。方法回顾性分析自2014年1月至2017年12月应用改良SP(改良SP组)和PKP(PKP组)治疗骨质疏松性椎体压缩骨折的患者。分别统计两组患者手术时间,术前、术后3天、术后3个月、术后6个月视觉模拟评分(VAS)评分,术前、术后3个月、术后6个月伤椎Cobb角,术前、术后3个月、术后6个月患者功能障碍指数(ODI)评分等指标,比较两种手术的止痛效果、后凸畸形改善以及患者功能恢复情况。结果两组共44例患者均顺利完成手术,无术中、术后并发症发生。两组患者平均手术时间分别为23.85±5.08 min和22.96±4.28 min,无统计学差异(P>0.05)。两组患者术后疼痛症状均明显缓解,ODI降低,伤椎Cobb角减少。两组患者术后3 d、3个月、6个月VAS评分差异均无统计学意义(P>0.05)。两组患者术后3个月、6个月ODI评分比较,差异均无统计学意义(P>0.05);两组患者术后3个月、6个月伤椎Cobb角差异均无统计学意义(P>0.05)。结论治疗骨质疏松性椎体压缩骨折,改良SP可以达到类似PKP的近期疗效,无论在缓解疼痛、纠正椎体后凸畸形还是改善患者生活质量方面,近期效果均满意。
Objective To compare the short-term effects of modified stentoplasty（ SP） and percutaneous kyphoplasty（ PKP） in the treatment of osteoporotic vertebral compression fractures. Methods Patients with osteoporotic vertebral compression fractures treated by modified SP or PKP were collected from January 2014 to December 2017. The operation time of two groups was recorded. The VAS score was collected preoperative,3 days after operation,3 month and 6 months follow-up. Cobb angle and ODI score were also collected. Results All 44 patients of the two groups were successfully completed surgery,and there were no intraoperative and postoperative complications. The average operation time of the two groups was 23. 85 ± 5. 08 min and 22. 96 ± 4. 28 min respectively,and there was no significant difference between two groups（ P〉0. 05）. The VAS score,ODIscore and cobb angle were significantly improved after opertioan than those before operation in two groups. There was no significant difference in the VAS score between the two groups at 3 days,3 months,and 6 months after operation. There was no statistical difference between two groups in terms of ODI scores and the Cobb angle of injured vertebrae at 3 months and 6 months after operation. Conclusion The improved SP can achieve similar short term effect as PKP,and the effect is satisfactory in alleviating pain,improving kyphosis or quality of life of patients.
