慢性胰腺炎患者血清miR-221和miR-130a表达水平及临床意义 被引量:2

摘要 目的探讨慢性胰腺炎(CP)患者血清miR-221和miR-130a表达水平及临床意义。方法选取四川省人民医院消化内科收治的CP患者60例作为研究组,同期健康体检合格者60例作为对照组,采用实时荧光定量PCR方法检测血清miR-221和miR-130a表达水平,进一步分析CP患者血清miR-221和miR-130a的表达水平、两者相关性以及临床意义。结果慢性胰腺炎组患者血清miR-221与miR-130a表达水平明显高于对照组,差异有统计学意义(P<0.05);慢性胰腺炎组IL-1β、IL-6、IFN-γ、TIMP-1水平明显高于正常对照组(P<0.05),两组IGF-1、IGFBP-4水平比较差异无统计学意义(P>0.05);CP患者血清miR-221a表达水平与IL-1β、IL-6、TIMP-1呈明显正相关(P<0.05),与IFN-γ、IGF-1、IGFBP-4无明显相关性(P>0.05);miR-130a表达水平与IL-6、IFN-γ呈显著相关性(P<0.05),与IL-1β、TIMP-1、IGF-1、IGFBP-4无明显相关性(P>0.05);慢性胰腺炎患者血清miR-221与miR-130a表达呈正相关(P<0.05);以miR-221、miR-130a表达水平绘制受试者工作曲线(ROC曲线),miR-221的曲线下面积(AUC)为0.947,诊断灵敏度为86.7%,特异度为93.3%;miR-130a的AUC为0.914,诊断灵敏度为81.7%,特异度为88.3%。结论 CP患者血清miR-221和miR-130a表达水平显著升高,miR-221和miR-130a呈明显正相关,可能在CP发生发展过程中具有相似功能,可作为CP早期诊断的潜在生物指标。 Objective To investigate the expression levels and clinical significances of serum miR-221 and miR-130a in patients with chronic pancreatitis( CP). Methods Sixty patients with CP who admitted to the Department of Gastroenterology of our hospital were selected as the study group,sixty patients who passed the physical examination during the same period were selected as the control group,the serum miR-221 and miR-130a expression levels were detected by real-time fluorescence quantitative PCR,further analyzed of CP patients with serum miR-221 and miR-130a expression levels,the correlation and clinical significance. Results The levels of serum miR-221 and miR-130a in patients with chronic pancreatitis were significantly higher than those in the control group( P〈0. 05); the levels of IL-1β,IL-6,IFN-γ and TIMP-1 in chronic pancreatitis group were significantly higher than those in normal control group( P〈0. 05),there was no significant difference in the levels of IGF-1 and IGFBP-4 between the two groups( P〉0. 05). The level of serum miR-221 a in patients with CP was positively correlated with IL-1β,IL-6,TIMP-1( P〈0. 05),and was not significantly correlated with IFN-γ,IGF-1,and IGFBP-4( P〉0. 05).The expression level of miR-130a was significantly correlated with IL-6 and IFN-γ( P〈0. 05),but not correlated with IL-1β,TIMP-1,IGF-1 and IGFBP-4( P〉0. 05). There was a positive correlation between serum miR-221 and miR-130a expression in patients with chronic pancreatitis( P〈0. 05). The ROC curves were plotted with miR-221 and miR-130a expression levels,the AUC of miR-221 was 0. 947,the diagnostic sensitivity was 86. 7%,and the specificity was 93. 3%. The AUC of miR-130a was 0. 914,the diagnostic sensitivity was 81. 7%,and the specificity was 88. 3%. Conclusion The levels of serum miR-221 and miR-130a in patients with CP are significantly increased,there was a significant positive correlation between miR-221 and miR-130a,they may have similar functions in the development of CP,it can be used as potential biological indicators for early diagnosis of CP.
