摘要
目的探讨基于不同单元结构的颅内动静脉畸形(AVMs)在行血管内介入治疗时栓塞剂的选择。方法回顾性连续纳入2015年1月至2017年12月在广东省人民医院神经外科行单纯血管内介入治疗的颅内AVMs患者163例,根据使用栓塞材料的不同,将其分为3组,即Onyx组(79例),为单独使用Onyx胶;Glubran 2组(55例),为单独使用Glubran 2胶;联合组(29例),为联合使用Onyx胶+Glubran 2胶。分析3组单元结构特点及治疗效果。结果 163例患者中男100例,女63例。AVMs的单元结构要素包括供血单元、引流单元和血管沟通。Onyx组、Glubran 2组及联合组栓塞治愈分别为33例(41.8%)、24例(43.6%)和10例(34.5%),栓塞治愈率的组间差异无统计学意义(χ~2=0.681,P=0.711)。总体栓塞治愈率41.1%(67/163),并发症发生率4.3%(7/163)。结论基于AVMs不同单元结构选择不同的栓塞剂均可获得较好的栓塞效果。
Objective To investigate the selection of embolic agents in intravascular interventional treatment for intracranial arteriovenous malformations(AVMs) based on different unit structures.Methods From January 2015 to December 2017,163 consecutivre patients with intracranial AVM received intravascular interventional therapy alone at the Department of Neurosurgery,Guangdong General Hospital were enrolled retrospectively. According to the use of different embolic materials,they were divided into 3 groups: Onyx group(n = 79; using Onyx glue alone),Glubran 2 group(n = 55; using Glubran 2 glue alone),and combination group(n = 29; using Onyx glue + Glubran 2 glue). The unit structure characteristics and therapeutic effects of the 3 groups were analyzed. Results Of the 163 patients,63 were female and100 were male. The unit structure elements of AVMs include feeding unit,drainage unit,and vascular communication. The curative rate of the Onyx group,Glubran 2 group and combination group were 41. 8%(n = 33),43. 6%(n = 24),and 34. 5%(n = 10),respectively. There were no significant difference in the curative rate of embolization among the groups(χ^2= 0. 681,P = 0. 711). The overall curative rate of embolization was 41. 1%(67/163),and the incidence of complication was 4. 3%(7/163). Conclusion Selection of different embolic agents based on unit structures of AVMs may obtain good effect of embolization.
