咖啡酸防治B细胞性非霍奇金淋巴瘤化疗后骨髓抑制的临床研究 被引量:7

Effect of caffeic acid on chemotherapy-induced hypocytosis in patients with B-cell non-Hondgkin's lymphoma

摘要 目的观察咖啡酸片防治B细胞性非霍奇金淋巴瘤患者化疗后骨髓抑制的r临床效果。方法选择接受2个疗程R-CHOP方案化疗的B细胞性非霍奇金淋巴瘤患者82例,随机分为对照组和观察组,每组41例。观察组患者自化疗当天开始口服咖啡酸片300mg/次,3次/d,至化疗后14d停止;对照组患者单纯化疗,不服用任何升血药物,当出现出现骨髓抑制时,再给予重组人粒细胞刺激因子注射液和(或)重组人白细胞介素-11,至血象正常。观察两组白细胞、血小板数及骨髓抑制和感染、出血发生率。结果两组患者化疗后白细胞、血小板数均有明显下降,化疗后7、14和21d各个时间点观察组白细胞数、血小板数均明显高于对照组,差异有统计学意义(P〈0.05)。观察组患者骨髓抑制的发生率为29.27%,明显低于对照组(60.98%);观察组患者Ⅲ度及以上骨髓抑制发生率为2.44%,明显低于对照组(17.07%,P〈0.05)。观察组感染发生率为24.39%,明显低于对照组(56.09%)。观察组患者出血发生率14.63%,明显低于对照组(36.59%),差异有统计学意义(P〈0.05)。结论咖啡酸片防治B细胞性非霍奇金淋巴瘤患者化疗后骨髓抑制所致的白细胞、血小板减少疗效显著。 Objective To investigate the clinical effect of caffeic acid tablets on hypocytosis induced by R-CHOP regimen chemotherapy on patients with B-cell non-Hondgkin' s lymphoma. Methods Eighty-two patients with B-cell non-Hondgkin lymphoma were randomly divided into two groups, with 41 patients in each group. Each patient received R-CHOP regimen chemotherapy for 2 courses, and each course lasted for 21 days. The patients in the observation group were treated by oral caffeic acid tablets 0.3 g/time, 3 times/d, until the 14th day after chemotherapy. And the patients in the control group were treated by chemotherapy only. The number of white blood cells (WBC), platelet (PLT), the incidence rate of bone marrow suppression, and the incidence of infection, hemorrhage were observed. Results The number of WBC, PLT at the 7th day, 14th day and 21th day after chemotherapy was significantly higher in the observation group than those of the control group ( P 〈 0.05 ). The incidence of bone marrow suppression of the observation group was 29.27% , which was significantly lower than that of the control group (60. 98% , P 〈 0. 05 ). The incidence of infection in the observation group was 24. 39% , which was significantly lower than that of the control group (56.09% , P 〈 0. 05 ). The incidence of hemorrhage in the observation group was 14.63%, which was significantly lower than that of the control group ( 36.59% , P 〈 0. 05 ). Conclusions Caffeic acid can effectively prevent and treat leukopenia and thrombocytopenia induced by chemotherapy in patients with B-cell non-Hondgkin' s lymphoma.
