摘要
目的观察采用脑室型颅内压监测下的精准规范化治疗双额叶脑挫裂伤的效果。方法双额叶脑挫裂伤患者320例均分为两组:研究组植入脑室型颅内压探头,实施精准规范化治疗;对照组无脑室探头植入,常规手术。比较两组治疗效果。结果研究组中转手术、脑中心疝、双侧开颅和去骨瓣病例均少于对照组(P<0.01)。与对照组比较,研究组住院时间较短,医疗费用较低(P<0.05)。伤后6个月,两组患者GOS评分相仿(P>0.05)。结论与常规治疗比较,脑室型颅内压监测能为创伤性双额叶挫裂伤患者的治疗提供客观依据,改善临床疗效。
Objective To observe the outcomes of precise and standardized treatment of bilateral frontal contusion guided by intraventricular pressure monitoring.Methods A total of 320 patients with bilateral frontal contusion was equally assigned into two groups.The patients in group A received precise and standardized treatment guided by intraventricular pressure monitoring and those in group B received conventional treatment.The outcomes were compared between two groups.Results The percentages of patients converting to surgery,occurring brain central hernia,experiencing bilateral craniotomy and removing bone flap were lower in group A than those in group B（P〈0.01）.Compared to group B,group A had shorter hospital stay and less medical expenses（P〈0.05）.The GOS scores of two groups at 6 months after treatment were similar（P〉0.05）.Conclusion Compared to conventional treatment,intraventricular pressure monitoring can provide objective evidence for the treatment of the patients with bilateral frontal contusion and improve clinical efficacy.
