摘要
目的探讨软肝汤在慢性肝炎后肝纤维化患者中的临床效果及其对T细胞亚群的影响。方法选择慢性肝炎后肝纤维化患者82例,随机均分为两组,分别采用多烯磷脂酰胆碱胶囊联合恩替卡韦片治疗(西药组)和软肝汤治疗(软肝汤组)。检测TBil、血清白蛋白(Alb)、ALT、AST、血清透明质酸(HA)、Ⅳ型胶原(Ⅳ-C)、Ⅲ型前胶原(PC-Ⅲ)及层黏蛋白(LN)水平。测定CD3^+、CD4^+、CD8^+及CD3^+/CD8^+T细胞亚群水平。记录不良反应发生情况。结果软肝汤组治疗后12周的TBil、ALT、AST、HA、Ⅳ-C、PC-Ⅲ及LN水平均低于西药组(P<0.05),而Alb水平无统计学差异(P>0.05)。软肝汤组治疗后12周的CD3^+、CD4^+及CD3^+/CD8^+T淋巴细胞亚群水平高于西药组(P<0.05),而CD8^+T淋巴细胞亚群低于西药组(P<0.05)。治疗后12周西药组不良反应发生率高于软肝汤组(P<0.05)。结论软肝汤对慢性肝炎后肝纤维化有明显缓解效果,提高患者T细胞亚群水平。
Objective To investigate the clinical efficacy of ruangan decoction in treating hepatic fibrosis after chronic hepatitis and its effect on T cell subsets.Methods Eighty-two patients with hepatic fibrosis after chronic hepatitis were randomly divided into two groups and treated with polyenylphosphatidylcholine capsules plus entecavir tablets（group A,41 cases）and Ruangan decoction（group B,41 cases）.Serum levels of TBil,albumin（Alb）,ALT,AST and hyaluronic acid（HA）,typeⅣ collagen（Ⅳ-C）,type Ⅲ procollagen（PC-Ⅲ）and laminin（LN）were detected.The levels of CD3^＋,CD4^＋,CD8^＋and CD3^＋/CD8^＋T cell subsets were determined and compared between two groups.The incidence of adverse responses in two groups on the 12 th week of treatment was observed.Results The serum levels of TBil,ALT,AST,HA,Ⅳ-C,PC-Ⅲ and LN in group B were lower than those in group A on the 12 th week of treatment（P〈0.05）,but there was no significant difference in Alb level of two groups（P〈0.05）.The levels of CD3^＋,CD4^＋and CD3^＋/CD8^＋T lymphocyte subsets were higher,while the CD8^＋T lymphocyte subsets were lower,in group B than those in group A on the 12 th week of treatment（P〈0.05）.The incidence of adverse responses in group A was higher than that in group B in 12 weeks of treatment（P〈0.05）.Conclusion Ruangan decoction treatment may effectively attenuate the hepatic fibrosis and elevate the level of T cell subsets in the patients with chronic hepatitis.
