摘要
目的探讨子宫内膜异位症(EMT)相关性不孕患者腹腔镜术后妊娠情况及影响因素,为有效提升患者妊娠率提供参考依据。方法选择2014年1月-2016年1月德州市妇幼保健院妇科收治的100例EMT合并不孕患者为研究对象,所有患者均采用腹腔镜手术治疗,采用电话约访及门诊复查的方式对患者术后1年内的妊娠情况进行随访,并通过查阅病例及面对面询问等方式收集患者的相关资料,包括年龄、既往妊娠史、EMT分期、输卵管功能、术前不孕病程、不孕类型、异位囊肿的患侧类别、术后接受助孕等情况,采用单因素及多因素Logistic方法分析相关因素对妊娠情况的影响。结果 100例EMT患者中,61例患者在术后1年内妊娠,妊娠率为61.00%;单因素及多因素Logistic分析结果显示,患者的年龄、EMT分期、输卵管功能、术前不孕病程及术后接受助孕措施均是EMT患者术后1年内妊娠率的影响因素(P<0.05),而既往妊娠史、异位囊肿的患侧类别及不孕类型均与术后妊娠率无关(P>0.05)。结论腹腔镜术治疗可有效提高EMT患者术后1年的妊娠率,且妊娠率的高低与患者的年龄、EMT分期、输卵管功能、术前不孕病程及术后接受助孕措施有关。
Objective To explore pregnancy of patients with endometriosis-associated infertility after laparoscopic surgery,analyze the influencing factors,provide a reference basis for effectively improving pregnancy rate. Methods From January 2014 to January 2016,100 patients with endometriosis-associated infertility were selected from Department of Gynecology in Dezhou Maternal and Child Health Care Hospital as research object,all the patients were treated by laparoscopic surgery,the pregnancy rates within one year after surgery were followed up by telephone interview and in Outpatient Department. The related data was collected by consulting the patients and face-to-face inquiry,including age,previous pregnancy history,endometriosis staging,tubal function,preoperative duration time of infertility,types of infertility,types of affected ectopic cyst,and postoperative assisted reproduction. Univariate and multivariate logistic models were used to analyze the effects of related factors on pregnancy rates. Results The pregnancy rate was 61. 00%（ 61/100）. Univariate and multivariate logistic models showed that age,endometriosis staging,tubal function,preoperative duration time of infertility,and postoperative assisted reproduction were influencing factors of pregnancy rate within one year after surgery（ P〈0. 05）,previous pregnancy history,types of affected ectopic cyst,and types of infertility were not correlated with pregnancy rate within one year after surgery（ P〈0. 05）. Conclusion Laparoscopic surgery can effectively improve pregnancy rate of patients with endometriosis-associated infertility within one year after surgery,which is correlated with age,endometriosis staging,tubal function,preoperative duration time of infertility,and postoperative assisted reproduction.
