导航下经皮微创附加伤椎内固定治疗胸腰椎骨折的临床疗效分析 被引量:3

Analysis of the clinical efficacy of minimally invasive percutaneous add injured vertebral internal fixation under navigation for the treatment of thoracolumbar fracture

摘要 目的分析经皮微创内固定治疗胸腰段骨折的临床疗效。方法选取我科2015年3月—2016年10月收治的胸腰椎骨折患者38例为研究对象,随机分为研究组和对照组,每组各19例。给予对照组传统复位内固定手术治疗,研究组患者接受经皮微创内固定治疗,比较两组术中和术后恢复指标、术后影像学指标以及并发症发生率情况。结果导航下经皮微创内固定术治疗明显降低其手术时间与术中出血量,减少X线暴露时间与住院治疗时间,差异有统计学意义;两种治疗方式均能改善患者椎体前缘高度比值、后凸Cobb’s角,研究组改善效果显著,相比对照组更佳,差异无统计学意义;微创组降低术后并发症发生率,预后效果更佳,P>0.05。结论经皮微创附加伤椎内固定治疗胸腰段脊柱骨折具有较好的治疗效果,能明显改善患者骨折状况,并发症少,安全性较高,值得临床推广与应用。 Objective To analyze the clinical effect of minimally invasive percutaneous add internal fixation in the treatment of thoracolumbar fracture. Methods 38 patients with spinal fractures during March 2015 and October 2016 were selected as the study subjects,and were randomly divided into the study group and the control group,and 19 cases in each group. The control group was treated by traditional open internal fixation. The patients in the study group were treated with minimally invasive percutaneous internal fixation. The intra-operative and postoperative recovery indicators,the postoperative imaging index and the complications incidence were compared between the two groups. Results Minimally invasive percutaneous internal fixation under the navigation,the operation time,intra-operative blood loss,the time of X-ray exposure and the time of hospitalization were significantly reduced,the differences were statistically significant. Both treatments could improve the vertebral height percentage and the postoperative sagittal Cobb's Angle. The improvement effect of the study group was better than that of the control group,but the difference was not statistically significant. The incidence of postoperative complications of the minimally invasive group significantly reduced,and the prognosis was better,the difference was statistically significant(P〈0. 05). Conclusions Minimally invasive percutaneous add injured vertebral internal fixation under navigation is a good treatment for thoracolumbar fractures,which can significantly improve the fracture status,and has advantages of fewer complications and high safety. It is worthy of clinical promotion and application.
