摘要
目的对影响颅脑损伤患者在术后出现迟发性颅内血肿的相关因素进行分析。方法对本院在2012年12月~2017年12月收治的130例颅脑损伤患者,所有患者均进行开颅清除血肿术,其中发生迟发性颅内血肿患者10例,为观察组;未发生迟发型颅内血肿为120例,为对照组。本文分析的影响因素包括了受伤机制、首次检查CT时间、颅脑损伤程度、是否出现开放性损伤、脑挫裂伤、颅骨骨折、血肿量、入院至手术时间以及去骨瓣减压术等。采用非条件多因素Logisitic回归分析。结果观察组患者的颅内损伤程度重度比例、脑挫伤比例、颅骨骨折比例、去骨瓣减压术比例均高于对照组患者。非条件的多因素Logisitic回归分析分析结果显示,重度颅脑损伤(OR=3.869)、脑挫裂伤(OR=2.894)、颅骨骨折(OR=1.895)以及去骨瓣减压术(OR=0.514)是引起患者术后迟发性颅内水肿的独立危险因素。结论重度颅脑损伤、脑挫裂伤、颅骨骨折以及去骨瓣减压术是影响术后迟发型颅内血肿的关键独立危险因素。
Objective To analyze the related factors of postoperative delayed intracranial hematoma in patients with craniocerebral injury. Meth- ods Atotal of 130 patients with craniocerebral injury who were treated in our hospital from December 2012 to December 2017 were enrolled in this study. All patients underwent craniotomy and hematoma removal. 10 patients with delayed intracranial hematoma were selected as observation group. No delayed intracranial hematoma was 120 cases, as the control group. The factors analyzed in this paper include the mechanism of injury, the time of CT examination for the first time, the degree of head injury, the presence of open injury, the contusion of the brain, the skull fracture, the amount of hematoma, the time between admission and operation, and the decompression of bony flap. Non-conditional multivariate Logisitic regres- sion analysis was used in this paper. Results The severity of intracranial injury in the observation group was significantly higher than that in the con- trol group. The proportions of brain contusion, proportion of skull fracture and decompressive craniectomy were all higher in the observation group. Non-conditional multivariate logistic regression analysis showed that severe craniocerebral injury (OR 3.869), cerebral contusion (OR 2.894), skull fracture (OR 1.895) and bony flap decompression (OR=0.514) is am independent risk factor for postoperative delayed intracranial edema. Conclusion Severe craniocerebral injury, cerebral contusion and laceration, skull fracture and debridement decompression are the key independent risk factors for postoperative delayed intracranial hematoma.
