摘要
目的:观察呼吸训练对颈段脊髓损伤患者咳嗽咳痰无力的疗效。方法:选取20名颈段脊髓损伤患者,全部为男性,年龄在25-60周岁,ASIA:C级,截瘫时间为2年或2年以上且病情稳定,无肺部损伤,主诉有日常咳嗽咳痰无力症状。将该20名患者随机分为观察组和对照组2组,对照组进行常规康复治疗;观察组在常规治疗的基础上给予呼吸训练,2组患者在完成2个月治疗后对肺功能指标进行检测比较。结果:2组患者在实验治疗前肺功能测定比较P〉0.05,无显著差异,经过2个月的呼吸训练后,2组患者的肺功能指标有显著性差异P〈0.05,观察组的肺功能结果明显优于对照组。结论:通过呼吸训练,可有效地维持和增大胸廓的活动度,增强呼吸肌的弹性,提高呼吸肌及辅助肌群的力量,使患者咳嗽咳痰更为颇畅,减少了因咳嗽费力和气道分泌物排出困难而导致肺部感染的危险。
Objective:To observe the effect of respiratory training on cough and expectoration weakness in patients with Cervical Spinal Cord injury.Methods:Selected 20 patients with cervical spinal cord injury,all male,between 25 and 60 years old,ASIA :C grade,2 years or more than 2 years in paraplegia and in a stable condition,no lung injury,complained of daily cough and expectoration weakness.The 20 patients were randomly assigned to 2 groups experimental group and control group,the control group was only treated with routine rehabilitation;the experimental group added respiratory training in addition.The pulmonary function of the 2 groups patients were analyzed and compared after 2 months treatment.Resltlts:The pulmonary function in the patients of two groups before test treatment p 〉 0.05,no significant difference.After 2 months of respiratory training,the indexes of pulmonary function in the patients of the two groups had signif-icant difference p 〈 0.05.The results of pulmonary function test showed that patients in the experimental group were significantly better than that of the control group.Conclusion:The respiratory training could effectively maintain and increase thoracic mobility,enhance the softness of the respiratory muscles,improve the strength of expiratory muscle,so that patients could cough and expectoration more smooth-ly,reduce the difficulty of cough and airway discharge,which may lead to lung infection.
