摘要
[目的]了解小型民营医疗机构医务人员的执业环境现状,分析存在的问题,探讨其影响因素,为改善医务人员执业环境,构建和谐医患关系提供借鉴。[方法]采用描述性统计、卡方检验和Ordinal回归分析法对南京市186家民营机构的557名医务人员进行问卷调查,内容包括患者对其尊重度、信任度、满意度的看法及对医患关系的评价。[结果]78.25%的被调查者认为受到患者尊重,77.17%的被调查者认为患者对其提供服务信任,91.99%的被调查者认为患者对其服务满意,51.01%的被调查者认为社会对其职业尊重一般;38.07%的被调查者认为社会地位低;17.97%的被调查者认为医患关系紧张,其中3.27%的被调查者认为医患关系很紧张;年龄、每周工作时间、机构类型、社会对其职业的尊重、社会地位、患者的信任度及患者满意度是影响医患关系评价的显著因素。[结论]政府应适当对小型民营医疗机构减免部分税收;严格规范其医疗行为,规定其利润提取比例;开展考核评定,促进其医疗技术提高。小型民营医疗机构应该在增加人力的同时,通过考核机制激励现有医务人员提高工作效率,降低相对工作时间和劳动强度。医务人员应该增加与患者的沟通,提高自身技术水平,提升医疗质量。
Objective To understand current situation of practice environment of medical personnel in small private medical institution,analyze existing problems,and explore its influencing factors,so as to provide reference for improving medical staff ' s practice environment and constructing harmonious doctor-patient relationship. Methods Questionnaire was conducted to 557 medical staffs from 186 private medical institutions by descriptive statistics,chi-square test and ordinal regression,the content included respect,trust,satisfaction degree of patient to doctor and evaluation of doctor-patient relationship. Results 78. 25% of the respondents believe that was respected by patient,77. 17% believe that patients trust their services,91. 99% believe that patients satisfied with their services,51. 0% believe that society respected in general for their profession,38. 07% believe that their social status was low,17. 97% believe that doctor-patient relationship was stressed,and 3. 27% believe it was very stressed. Age,working hour every week,organization type,social respect for their profession,social status,patient' s trust and satisfaction degree were significant factors that affected evaluation of doctor-patient relationship. Conclusions The government should derate part of tax to small private medical institutions. Strictly regulate their medical behavior and profit extraction proportion. Carry out assessment and evaluation,to promote their medical technology. Small private medical institutions should increase manpower,at the same time,through the assessment mechanism to encourage medical staff to improve work efficiency,reduce relative working hours and labor intensity. Medical staff should increase communication with patients,improve their own technical level,promote the quality of medical care.
