摘要
目的:探讨关节镜下改良Endobutton结合锚钉治疗肩锁关节脱位的标准化康复与护理方法。方法:对32例Ⅲ~Ⅴ型急性肩锁关节脱位患者进行关节镜下改良Endobutton结合锚钉双重解剖重建技术重建喙锁韧带的患者,术后制定系统的标准化的康复与护理方法,定期随访,并采用美国肩肘协会评分系统ASES评分,Constant-Murley肩关节功能评分及SST问卷评分,评定疗效。结果:全组患者术后随访12个月随访,VAS、ASES及Constant-Murley评分治疗前后比较,差异有统计学意义(P<0.05)。所有患者肩关节活动基本恢复正常,平均上举约(165.62±15.50)°,外旋(45.18±9.26)°。优良率93.7%(30/32)。结论:肩锁关节脱位患者应用改良Endobutton结合锚钉双重解剖重建技术重建喙锁韧带,术后系统的标准化的康复与护理对肩关节功能恢复起着积极重要的作用。
Objective： To discuss standardized rehabilitation and nursing methods for the patients suffering dislocation of acromioclavicular joint treated by improved endobutton combined with rivet under the arthroscope.Methods： All 32 patients with dislocation of acromioclavicular joint at the phases of Ⅲ ~ Ⅴ accepted improved endobutton combined with rivet double anatomical reconstruction technique under the arthroscope to rebuild coracoclavicular ligament, and systematic standardized rehabilitation and care should be established after the operation, the patients were followed up regularly, ASES scores, Constant-murley shoulder joint functional scores and SST questionnaires were used after the surgery to assess clinical effects. Results： All the patients were followed up within 12 months after the surgery, when VAS, ASES and Constant-Murley scores were compared before and after the treating, there was a significant difference（P 〈0.05）. Shoulder motion of all the patients restored to normal,average lifting angle about（165.62 ±15.50） °, lsterotorsion（45.18 ±9.26） °, excellent rate was 93.7%（30/32）.Conclusion： The patients with dislocation of acromioclavicular joint accept improved Endobutton combined with rivet double anatomical reconstruction technique under the arthroscope to rebuild coracoclavicular ligament,postoperative systematic standardized rehabilitation and nursing methods are playing the positive and important roles in the recovery of shoulder function.
